Editorial commentary
Biosafety Level 5 (BSL-5) is not a real or recognized biosafety designation. The standard US biosafety containment framework — the CDC/NIH Biosafety in Microbiological and Biomedical Laboratories (BMBL, 6th edition, 2020) — defines four cumulative containment levels, BSL-1 through BSL-4, and stops there. BSL-4 is the maximum standard containment level currently defined for laboratory work with biological agents. This page exists because “Biosafety Level 5” is a search term people genuinely use, usually while trying to work out whether it is real — the short answer is no, and this page explains why the confusion arises and what the correct terminology actually is.
Why BSL-4 is the ceiling, not BSL-5
BMBL’s four levels are cumulative: each level layers additional practices, safety equipment, and facility engineering controls on top of the level below it, rather than replacing it. BSL-4 sits at the top of that stack because it is designed around the worst realistic case the framework addresses: agents that cause severe or fatal disease, for which no vaccine or treatment is routinely available, and where aerosol transmission is a credible exposure route (Ebola and Marburg virus are the commonly cited examples; Variola virus, which causes smallpox, is also handled at this level but is retained only at two WHO-authorized repositories rather than distributed to research laboratories generally). Because BSL-4 already assumes the most severe combination of risk factors the framework accounts for, there is no defined scenario in BMBL that a BSL-5 would exist to address — the containment measures BSL-4 requires (dedicated isolated facilities, redundant HEPA-filtered air handling, positive-pressure suits or Class III cabinet lines, mandatory decontamination showers) are already the maximum the standard specifies.
Where the “BSL-5” idea comes from
A few real, adjacent concepts likely feed the misconception:
- BSL-4 has two different primary-containment approaches, which can read as if they were separate levels: a cabinet laboratory, where all agent work happens inside a gas-tight Class III biosafety cabinet, and a suit laboratory, where personnel work at an open bench wearing a full-body, positive-pressure, HEPA-filtered supplied-air suit with a mandatory chemical shower before exiting. Both are BSL-4 — neither is a higher numbered tier.
- Institution- or agent-specific “enhanced” protocols are sometimes layered on top of baseline containment for a small number of especially high-consequence pathogens or procedures, at the discretion of an Institutional Biosafety Committee (IBC) or a funder. These enhancements are documented as BSL-3 “enhanced” or BSL-4-plus practice additions in institutional biosafety manuals, not as a new numbered level in BMBL itself.
- Informal and media usage. Journalism, fiction, and casual conversation about outbreaks or bioterrorism sometimes reach for “level 5” to signal “worse than the worst officially defined category,” without the phrase corresponding to any actual regulatory or scientific designation.
