An R33 is an NIH research-grant activity code — part of the R-series, the same family as R01 and R21 — that funds the second, expanded-development phase of a milestone-driven, two-phase award. R33 support is almost never awarded on its own: it is typically paired with a first exploratory phase, either an R21 (the combined “R21/R33” mechanism) or, in newer and clinical-trial-oriented Funding Opportunity Announcements (FOAs), an R61 (the “R61/R33” mechanism). An applicant submits a single application describing both phases; funding of the second (R33) phase is contingent on the awarding NIH Institute or Center (IC) confirming that predefined, quantifiable go/no-go milestones from the first phase were met — not on a second competitive application.

The R21/R33 and R61/R33 phased structure

In the R21/R33 model, the R21 phase (up to two years, capped NIH-wide at $275,000 in total direct costs with no more than $200,000 in any single year) supports feasibility work, proof-of-concept, or generation of critical preliminary data. If the applicant meets the milestones defined in the original application, NIH staff transition the award into the R33 phase — typically up to three years — to support expanded development, validation, or optimization of the approach, without requiring a new competing application. In the newer R61/R33 model (used heavily in NIH’s translational and early-phase clinical-trial FOAs), the R61 phase plays the same exploratory role the R21 plays above, and program staff conduct a formal administrative review of the R61 milestones before the R33 phase is allowed to proceed. Duration and budget for the R33 phase itself are not capped NIH-wide the way the R21 is — each governing FOA sets its own ceiling, so the actual numbers for any given R33 phase come from that FOA, not from a fixed NIH rule.

R33 is a grant, not a cooperative agreement

The R33 activity code is frequently mislabeled online as an “NIH Exploratory/Developmental Cooperative Agreement.” That is inaccurate: per NIH’s own activity-code definitions, R33 sits in the R series (Research Projects) — a standard research grant, administered with the same investigator independence as an R01 — not the U series, which is where NIH’s actual cooperative agreements live (U01, U19, and similar codes). The distinction matters administratively, not just semantically: under a grant, the investigator has primary responsibility for developing the concepts, methods, and approach; under a cooperative agreement, NIH program staff are substantially involved in study design, data collection, or coordination on an ongoing basis. An R33 award does not carry that substantial-involvement obligation. The confusion likely comes from R33’s role inside a milestone-reviewed phased mechanism, which superficially resembles the staged, NIH-monitored structure of some cooperative agreements — but the funding instrument itself is a grant.

How R33 differs from a standard R01

Both R33 and R01 are R-series research grants with the same basic investigator-independence model, but they serve different purposes. An R01 supports a discrete, already-de-risked research project chosen and defined entirely by the investigator, typically requiring supporting preliminary data at application. An R33 exists specifically to fund the development phase of work whose feasibility was established in a prior, formally milestone-reviewed exploratory phase — it is not awarded to a first-time applicant with no prior R21/R61 progress except under specific FOAs that explicitly accept equivalent outside preliminary data. In practice this means R33 project narratives are built around a go/no-go milestone framework NIH staff evaluate mid-project, where an R01’s progress is typically reviewed only at renewal.

Typical use cases

The R21/R33 and R61/R33 mechanisms exist for high-risk, high-reward research that does not fit a traditional R01 timeline: novel assay or biomarker development, early-stage therapeutic or device development, and other work where NIH wants evidence of feasibility before committing multi-year development funding, but also wants to avoid the 12–18 month gap and administrative restart of a fully separate competing application between an exploratory finding and its development. Sponsored-programs offices should treat a combined R21/R33 or R61/R33 award as a single grant number with two internal budget/reporting phases, not two separate awards — the go/no-go decision point is an internal NIH administrative action, not a new Notice of Award requiring a fresh proposal routing.
