A construction site is not a warehouse or an office building with a first aid cabinet bolted to a break-room wall. There is usually no infirmary down the hall, no HR office to page, and frequently no fixed address at all yet — the site itself is the address. OSHA’s construction standard, 29 CFR 1926.50, was written around that reality: it assumes some sites will not have a hospital, clinic, or physician “reasonably accessible” in time or distance, and it builds the first-aid-station requirement around covering that gap, not just around having a kit on a wall. Combine that access problem with a hazard profile dominated by falls, power-tool and sharp-material lacerations, and struck-by or caught-in/between crush injuries, and a construction first aid station has to be stocked and staffed differently than a general office one — not just bigger, but weighted toward stopping severe bleeding and stabilizing trauma in the minutes before EMS can reach an active site.

What OSHA 1926.50 Actually Requires

1926.50 is short, and every clause of it matters for how a site actually gets stocked and staffed:

  • (a) Medical personnel availability. The employer must ensure medical personnel are available for advice and consultation on matters of occupational health — a standing relationship with a clinic or occupational-medicine provider, not something arranged after an injury happens.
  • (b) Prompt medical attention arranged in advance. Before work begins, the employer has to have arranged for prompt medical attention in case of serious injury — know which hospital, how far, and how the injured worker gets there.
  • (c) A certified first-aid person when a facility isn’t reasonably accessible. In the absence of an infirmary, clinic, hospital, or physician reasonably accessible in terms of time and distance, a person holding a valid first-aid certificate (from a U.S. Bureau of Mines-accepted program or the American Red Cross, or equivalent training) must be available at the worksite.
  • (d) Supplies, easily accessible, in weatherproof containers. First aid supplies must be easily accessible, kept in a weatherproof container, with individual sealed packages for each type of item, and the contents checked before being sent out on each job and at least weekly thereafter to replace used or expired items.
  • (e) Transportation or a communication system. The employer must provide equipment for the prompt transportation of an injured worker to a physician or hospital, or a functioning communication system for contacting an ambulance service.
  • (f) Posted emergency information. In areas without reliable 911 service, the telephone numbers of physicians, hospitals, and ambulances must be posted; where 911 is available, the site needs an effective communication system and a way to identify the worksite’s exact location (address or coordinates) to responders.
  • (g) Quick-drenching facilities for corrosive exposure. Where employees’ eyes or bodies may be exposed to corrosive materials, suitable facilities for quick drenching or flushing must be provided within the work area for immediate use.

Compare that to OSHA’s general workplace first aid kit standard (1910.151, the general-industry counterpart): the “reasonably accessible” and trained-responder logic is nearly identical, but 1926.50 adds explicit weatherproofing, per-job and weekly inspection cadence, and the corrosive-exposure drenching clause — all specific to a site that moves, changes shape week to week, and sits outdoors.

“Reasonably Accessible” Is the Clause That Decides Everything Else

Nothing in 1926.50 sets a hard mileage or minute figure for “reasonably accessible” — OSHA has consistently treated it as a fact-specific determination based on both distance and realistic travel time, not just what a map shows. That ambiguity is exactly why so many contractors default to staffing a trained first-aid responder on every shift rather than betting on off-site medical access: for a severe bleed, the margin for error is measured in minutes, and a site fifteen minutes from the nearest trauma center by road may be effectively unreachable in time to matter for a major hemorrhage. Treat “is a hospital reasonably accessible” as a question you answer conservatively, in writing, before the job starts — not a judgment call made after someone is hurt.

Why the Construction Risk Profile Is Different From an Office or Retail Floor

OSHA’s own construction fatality data has long pointed to what the industry calls the “Fatal Four”: falls, struck-by-object incidents, caught-in/between hazards, and electrocution — together responsible for the large majority of construction worker deaths. The non-fatal injury pattern that a first aid station has to absorb every week is the same family of hazards at lower severity: lacerations from power tools, rebar, sheet metal, and glass; puncture wounds; fall-related fractures and sprains; crush and pinch injuries from materials, equipment, and machinery; and burns from welding, cutting, hot surfaces, and electrical work. That’s a materially different profile from an office first aid station built around minor cuts, headaches, and the occasional allergic reaction — and it’s why a construction station needs to be built around trauma and hemorrhage readiness first, general comfort-care items second.

Hemorrhage Control: The Core of a High-Risk-Site Kit

A power-tool or rebar laceration, a degloving injury from machinery, or an amputation from a caught-in/between incident can produce uncontrolled bleeding that becomes life-threatening within minutes — well inside the window before most sites can realistically get an injured worker to a hospital. A construction-grade station should carry, at minimum:

  • At least one commercially manufactured tourniquet (CAT-style or equivalent), stored where it is immediately visible and reachable, not buried under general supplies
  • Hemostatic gauze for wounds a direct-pressure dressing alone can’t control
  • Pressure dressings/compression bandages sized for large wounds, not just small adhesive bandages
  • Trauma shears, for cutting away clothing or PPE quickly without adding a second sharp-object hazard
  • Nitrile gloves in quantity — bloodborne pathogen exposure risk is real and constant on a site producing this volume of lacerations

Burn Care: Welding, Electrical, and Chemical Exposure

Welding, cutting, hot-work permits, and live electrical work put thermal and electrical burns on the list of injuries a construction first aid station has to be ready for in a way most office stations never need to consider. Eye and Skin Flushing for Corrosive Exposure — 1926.50(g)

Concrete, mortar, masonry cleaners, and various construction chemicals are corrosive or caustic on contact with eyes and skin, which is exactly what 1926.50(g) is written around: suitable quick-drenching or flushing facilities have to be available within the work area itself, not back at a trailer five minutes away. A portable eyewash station or drench hose, positioned at the point of use rather than centralized, is the practical way to meet this on a site where the “work area” moves as the job progresses.

Weatherproofing and Container Requirements

1926.50(d) is explicit that supplies sit in a weatherproof container with individually sealed packages, and that contents get checked before each job and at least weekly after that. That is a materially higher bar than a metal wall cabinet in a climate-controlled office: heat, cold, humidity, and dust all degrade adhesive bandages, gauze packaging, and tape over a multi-month job. A sealed, gasketed plastic case rather than an open-shelf metal cabinet, and a real weekly inspection log rather than an assumption that nobody’s touched the kit, are both worth treating as compliance requirements, not nice-to-haves.

Sizing: One Station Usually Isn’t Enough on a Spread-Out Site

A single first aid station makes sense for a small, compact job. Trained First-Aid Responders and Certification

1926.50(c) requires a person with a valid first-aid certificate on-site whenever a medical facility isn’t reasonably accessible — acceptable training comes from the U.S. Bureau of Mines or the American Red Cross, or an equivalent program covering the same core skills. For a site running multiple shifts or multiple simultaneous crews, that means coverage per shift and per crew, not one certified person for the whole project regardless of who’s actually on-site at a given hour. Track certification expiration the same way PPE and equipment inspections get tracked — a lapsed card discovered after an injury is not a defensible position.

Construction Site Station vs. Corporate Office Station: Why the Difference Matters
