A materials manager comparing reusable scrub sets to disposable ones is usually looking at the wrong two numbers. Purchase price per garment tells you almost nothing about what the item actually costs a facility or a lab over its working life — reusable apparel carries a laundering and infrastructure cost that never shows up on the packing slip, and disposable apparel carries a waste-stream cost that only shows up in specific, contamination-triggered circumstances, not automatically every time an item is thrown away. This guide works through both cost models properly — cost-per-wear for reusables, cost-per-use for disposables — and then separates that economics question from a different one entirely: the scenarios where the choice isn’t a cost decision at all, because a regulation or an infection-control protocol has already made it for you.
Supplies, Not Equipment: How This Purchase Is Classified Under 2 CFR 200
For a lab or clinic operating under a federal award — NIH, SBIR/STTR, or any other agency grant subject to the Uniform Guidance — scrubs and coveralls are almost always supplies, not equipment, regardless of whether they’re reusable or disposable. Under 2 CFR 200.1’s definitions, “equipment” means tangible personal property with a useful life of more than one year and a per-unit acquisition cost at or above the non-federal entity’s capitalization threshold or $10,000, whichever is lower — a threshold raised from $5,000 in OMB’s April 2024 Uniform Guidance revision, effective for awards issued on or after October 1, 2024. “Supplies” is everything else: tangible personal property below that cost line, and 2 CFR 200.314 applies the same $10,000 figure (also raised from $5,000 in the same revision) to how residual supply inventory is handled at the end of a project.
A reusable scrub set’s multi-year service life doesn’t change this. What matters for capitalization is per-unit acquisition cost, not how many times the garment gets worn — a $12 scrub top laundered 100 times is still a $12 supply, not depreciable equipment. The Real Cost-Per-Wear Math for Reusable Apparel
Cost-per-wear is the only honest way to price a reusable garment, and it has more line items than the purchase order suggests:
- Acquisition cost — the per-unit price of the scrub set or coverall, typically lower per garment than a comparable disposable item because the fabric is built for repeated processing.
- Laundering cost per cycle — either a contracted linen service’s per-pound or per-piece processing rate, or, if the facility launders in-house, the amortized cost of the washer-extractor and dryer capacity, water, energy, detergent/disinfectant, and the labor to sort, wash, fold, and redistribute. This is the cost component a disposable comparison omits entirely, and it’s rarely small once labor is counted honestly.
- Garment lifespan — the number of industrial-launder cycles a garment survives before fabric integrity, color, or fit degrades past use. This varies by fabric blend and is manufacturer-rated; get the actual published cycle rating for the specific garment being sourced rather than assuming a generic figure, since it drives the denominator in the entire cost-per-wear calculation.
- Attrition and shrinkage — garments lost to staff turnover, damage beyond repair, or simple non-return are a real, ongoing cost that inflates effective cost-per-wear above the laundering-cycle math alone. Programs with high staff turnover or multi-site distribution see materially higher attrition than a single stable unit.
- Tracking infrastructure — many hospital linen-management programs embed RFID tags in reusable garments specifically to control attrition and automate reorder triggers; that tracking system is its own line item, not free.
The formula, stated plainly: cost per wear = (acquisition cost + [laundering cost per cycle × number of cycles used]) ÷ number of wears actually obtained. A reusable program only beats disposable economics when the denominator (real wears obtained before retirement or loss) is high enough to absorb the laundering and infrastructure cost — which is a genuinely different answer in a low-turnover outpatient clinic than in a high-turnover teaching hospital.
The Real Cost-Per-Use Math for Disposable Apparel
Disposable scrubs and coveralls trade the laundering line item for a simpler per-unit purchase price, but the full cost-per-use picture still has components worth pricing out:
- Unit price at volume — the direct comparator to a reusable garment’s acquisition cost, usually lower per single wear on paper, but only because it carries zero reuse.
- Procurement and replenishment burden — disposables require continuous reordering rather than a one-time (or periodic) reusable-fleet purchase. For a lab buying under a federal award, recurring disposable-apparel purchases accumulate toward 2 CFR 200.320’s procurement-method thresholds — the micro-purchase threshold ($15,000, effective October 1, 2025, self-certifiable up to $50,000/year with documented internal risk-assessment procedures) and the simplified acquisition threshold ($350,000) — in a way a single large reusable-fleet order may not. Track cumulative annual spend with a given vendor, not just the individual PO amount.
- Storage footprint — a facility running fully on disposables carries more standing inventory volume than one that launders and recirculates a fixed garment count.
- Waste-stream cost — but only when it’s actually triggered. This is the piece most buying comparisons get wrong. A used disposable scrub or coverall is not automatically regulated medical waste. OSHA’s Bloodborne Pathogens Standard, 29 CFR 1910.1030(b), defines “regulated waste” narrowly: liquid or semi-liquid blood or other potentially infectious materials (OPIM); items that would release blood or OPIM in liquid or semi-liquid form if compressed; items caked with dried blood or OPIM capable of releasing it during handling; contaminated sharps; and pathological/microbiological waste containing blood or OPIM. Per CDC guidance interpreting this same standard, most PPE and apparel used during standard care or transmission-based precautions does not meet that definition and can go into routine, non-infectious waste. The expensive biohazard-hauling cost only applies when a specific contamination event meets the regulated-waste test above — not as a blanket property of “disposable.” Facilities that route all used disposable apparel into red-bag waste by default are usually overpaying for a waste-classification decision the regulation never required.
When Disposable Isn’t a Preference — It’s the Compliance Requirement
The cost math above assumes the buyer has a real choice. In several recurring scenarios, it isn’t a choice:
- Single-use PPE cannot be reprocessed once designated single-use. 29 CFR 1910.1030(d)(3)(ix)(B) is explicit for disposable gloves — “disposable (single use) gloves shall not be washed or decontaminated for re-use” — and the same operating principle governs any garment an employer’s exposure control plan designates single-use under 1910.1030(d)(3)(xi)/(xii), which require gowns, aprons, lab coats, caps, hoods, and shoe covers to be selected by task and degree of exposure anticipated, including where gross contamination is anticipated. Once that task-based determination puts a garment in the single-use category, laundering it for reuse isn’t a cost-saving option — it’s a standard violation.
- A garment penetrated by blood or OPIM must come off immediately. 1910.1030(d)(3)(vi) requires removal “immediately or as soon as feasible” once penetration occurs, and (d)(3)(vii) requires all PPE to be removed before leaving the work area, with removed items placed in a designated container per (d)(3)(viii). That sequence assumes the garment is now either a decontamination event (for a reusable item cleared for reprocessing) or waste (for a single-use item) — not something worn again as-is either way.
- Isolation and transmission-based precautions frequently mandate single-use gowns per room or per resident specifically because within-shift reprocessing isn’t fast or clean enough to prevent cross-contamination between patients. - Cleanroom gowning is a different compliance driver entirely — particulate control, not bloodborne-pathogen exposure. In every one of these scenarios, the reusable-vs-disposable decision has already been made by the task, the exposure risk, or the room classification. The cost-per-wear math in the sections above only applies to the apparel categories left over once these compliance-mandated single-use requirements are carved out.
Justifying Either Choice on a Federal Award: 2 CFR 200.403
Where a real choice does exist, either a reusable or a disposable apparel program is allowable on an NIH, SBIR/STTR, or other federal award under 2 CFR 200.403’s cost-principle test: the cost must be necessary and reasonable for the project (200.404’s “prudent person” standard, applied at the time the purchasing decision was made), allocable to the award in proportion to benefit received (200.405), conform to any award-specific limitations, follow GAAP, and — the part most often missed — be treated consistently. 200.403 specifically requires that a cost not be charged as a direct cost on one award if the institution treats that same type of cost as indirect (F&A) elsewhere. Practically: if a lab’s home institution already recovers general laundry/linen costs through its negotiated F&A rate for its non-grant-funded operations, that same lab generally cannot turn around and direct-charge grant-funded reusable-scrub laundering as a distinct line item without specific award justification for why this circumstance is different. Confirm the institution’s existing direct-cost/indirect-cost treatment of laundering and apparel before building either program’s cost into a grant budget, and document the reasoning — 200.403’s adequate-documentation requirement applies here like any other cost decision.

A Practical Decision Framework
Work through these in order — the first two are compliance gates, not cost questions:
- Is the item designated single-use by an exposure control plan, isolation-precaution protocol, or cleanroom classification for the specific task? If yes, the decision is made — source disposable and stop the cost comparison there.
- Is contamination risk high enough that within-shift or between-patient reprocessing isn’t operationally feasible? If yes, disposable is the safer and usually the compliant default even absent an explicit single-use mandate.
- If neither gate applies, run the cost-per-wear vs. cost-per-use math above using this specific facility’s real laundering-contract rate (or in-house infrastructure cost), realistic attrition rate, and actual purchase volume — generic industry figures from a vendor’s marketing page are not a substitute for the facility’s own numbers.
- Check the federal-award consistency question (2 CFR 200.403) before building either program’s cost into a grant budget, not after an auditor asks about it.

Frequently Asked Questions
Are reusable or disposable scrubs cheaper overall?
Neither is cheaper in the abstract — it depends on real laundering-contract rates, attrition, and volume at the specific facility. High-volume, low-attrition, low-contamination settings tend to favor reusable once laundering infrastructure is already in place; low-volume or high-contamination settings, or any setting without an existing laundering contract, tend to favor disposable once the true infrastructure cost of starting a reusable program is counted.
Is used disposable PPE automatically regulated medical waste?
No. Under 29 CFR 1910.1030(b), “regulated waste” is defined narrowly — liquid/semi-liquid blood or OPIM, items that would release it if compressed, dried-blood-caked items capable of releasing it, contaminated sharps, and blood/OPIM-containing pathological waste. Most disposable apparel used in routine or standard-precaution care does not meet that definition and can go into regular waste; the biohazard-disposal cost applies only when an actual contamination event meets the regulated-waste criteria.
Can NIH or SBIR grant funds pay for reusable scrub laundering?
Potentially, but only if it passes 2 CFR 200.403’s consistency test — the institution can’t direct-charge a cost type to one award that it treats as indirect (F&A) elsewhere without specific justification. Confirm how the institution currently classifies laundry/linen costs before budgeting it as a direct grant expense.
Can a reusable scrub or coverall just be laundered instead of thrown away once it’s blood-contaminated?
Only if it was designated for reprocessing in the first place and the contamination doesn’t meet the regulated-waste or single-use-mandate criteria above. A garment an exposure control plan designates single-use — or one that meets OSHA’s regulated-waste definition — is not a laundering candidate regardless of the fabric it’s made from; 1910.1030(d)(3)(vi) requires immediate removal once penetrated, and reprocessing a garment the plan designates single-use is a standard violation, not a cost decision.
Are single-use cleanroom coveralls regulated the same way as isolation gowns?
No — different compliance driver. Cleanroom single-use requirements exist to control particulate and fiber shedding for the classified space, not bloodborne-pathogen exposure; isolation-gown single-use requirements exist under the Bloodborne Pathogens Standard and transmission-based precautions. A facility writing its own apparel policy should cite the correct standard for each context rather than treating them as one rule.
