PICOTS Criteria

A research question is PICOTS-structured when it explicitly specifies all six elements — Population, Intervention, Comparator, Outcome, Timing, and Setting — rather than only the four PICO elements, typically to derive study-eligibility criteria for a systematic review, evidence synthesis, or health-technology assessment protocol.

Editorial commentary

PICOTS (also written PICOTs) is an extension of the PICO framework used to structure a clinical or health-research question for a systematic review, evidence synthesis, or health-technology assessment. It adds two elements, Timing and Setting, to the original four (Population, Intervention, Comparator, Outcome), producing six discrete criteria that define exactly what evidence a review is looking for and, by extension, what its eligibility criteria for included studies should be.

The six elements

  • Population — the patients or subjects of interest (condition, demographics, care setting-relevant characteristics).
  • Intervention — the exposure, treatment, test, or policy being evaluated.
  • Comparator — the alternative(s) the intervention is being evaluated against (placebo, usual care, an active comparator, or no comparator).
  • Outcome — the measures used to judge effect (clinical endpoints, patient-reported outcomes, harms).
  • Timing — the duration of exposure or follow-up needed for outcomes to occur or be measured.
  • Setting — the clinical, geographic, or care context in which the intervention is delivered (primary care, inpatient, community, a specific health system).

Operational definition

A research question is a PICOTS-structured question when it explicitly specifies all six elements, not just the population/intervention/comparator/outcome (PICO) subset. In practice, review teams use PICOTS to (1) scope and word the review question, (2) derive the inclusion/exclusion criteria for the literature search, and (3) pre-specify these same criteria in a protocol — for example when registering a systematic review protocol on PROSPERO, or when reporting the review’s methods against the PRISMA 2020 checklist, both of which expect eligibility criteria to be stated up front and applied consistently.

Origin

The PICOTS variant is most closely associated with the U.S. Agency for Healthcare Research and Quality (AHRQ) Evidence-based Practice Centers (EPC) Program, which uses it in its Methods Guide for comparative effectiveness reviews and medical test reviews to make review questions and applicability judgments more explicit than PICO alone allows. Related variants exist in the literature (PICOTT, PCC for scoping reviews, PECO for exposure-based questions) but PICOTS — adding Timing and Setting — is the most widely used extension in evidence synthesis.

Worked examples

Example 1 (comparative effectiveness review). Among adults with non-cancer chronic pain (Population), does an opioid analgesic (Intervention), compared with a non-opioid analgesic or placebo (Comparator), improve pain and functioning (Outcome) when measured within three months of treatment (Timing) in outpatient settings (Setting)? This is the worked example AHRQ’s EPC Program guidance uses to illustrate the framework.

Example 2 (systematic review protocol). A review team registering a protocol on PROSPERO for a review of a diagnostic test would specify: patients with suspected condition X (Population), the index test (Intervention), the existing reference-standard test (Comparator), diagnostic accuracy measures such as sensitivity/specificity (Outcome), test result availability within a defined window (Timing), and primary versus specialist-care testing environments (Setting) — six criteria the eventual PRISMA 2020-reported review can be checked against.

Counter-example

A review question written only as “Does drug A work better than drug B for condition C?” specifies Population, Intervention, Comparator, and (implicitly) Outcome, but not Timing or Setting — this is a PICO question, not a PICOTS one. It becomes PICOTS only once the review team also states how long outcomes are followed and in what care setting the comparison applies, which is what lets reviewers judge whether a given study’s evidence actually transfers to the population and context the review is meant to inform.

Why it matters for evidence synthesis

Because Timing and Setting are exactly the dimensions most likely to limit how far a study’s results generalize, omitting them from a review’s eligibility criteria is a common source of scope creep and heterogeneity during study selection. Structuring the question with all six PICOTS elements before searching the literature — and stating them in a registered protocol — makes eligibility decisions reproducible across reviewers and gives readers of the completed, PRISMA-2020-reported review a clear basis for judging applicability of the findings to their own setting.
