Clinical Research Fellowships: Training Pathways and Certification Tracks

A guide to clinical research training fellowships and career-development pathways — T32/TL1, KL2, K23, degree programs like NIH-Duke, and ACRP/SOCRA certification — and how they differ.

A “clinical research fellowship” search covers two genuinely different things, and confusing them is the most common wrong turn: a clinical (subspecialty medical) fellowship trains a physician in a clinical discipline, while a clinical research training fellowship trains someone — physician or non-physician — in how to design, conduct, and analyze clinical research. This guide covers the second category: the funding mechanisms, degree programs, and workforce certifications that build clinical research competency, and how they relate to each other.

Academic, NIH-funded training pathways

Most academic clinical research training in the US runs through NIH mechanisms, layered by career stage. These are grants and awards, not employment contracts, and most require institutional sponsorship and a named mentor.

Institutional and individual NRSA training (T32, TL1, F32)

NIH’s T32 institutional training grants fund a department or program, not an individual, to train a cohort of predoctoral and postdoctoral fellows — some T32 programs are explicitly scoped to patient-oriented and clinical research rather than laboratory science. Within the NIH Clinical and Translational Science Awards (CTSA) Program, led by the National Center for Advancing Translational Sciences (NCATS), the TL1 mechanism is the training-grant component specifically built for clinical and translational research: it funds predoctoral candidates, combined health-professional doctorate-master’s candidates, and postdoctoral fellows seeking a practical introduction to clinical and translational research, and supports more than 500 trainees a year across CTSA hubs. An individual pursuing postdoctoral research training in a clinical research setting can also apply for an F32 Individual Postdoctoral Fellowship with a clinical-research-focused research plan and sponsor, rather than going through an institutional T32/TL1 slot.

Mentored career development awards (KL2, K23, K12)

Career development (K) awards are not “fellowships” in the training-grant sense, but they function as the next rung for someone who has finished formal training and needs protected time to build an independent clinical research career. Within CTSA hubs, the KL2 Mentored Career Development Award supports 2-5 years of protected time and mentored research career development for scholars — drawn from medicine, dentistry, nursing, behavioral science, biostatistics, and epidemiology — who have recently completed professional training and are starting basic, translational, or clinical research. Outside the CTSA structure, the standard NIH mechanism for a clinically trained investigator whose research involves direct interaction with human subjects is the K23 (Mentored Patient-Oriented Research Career Development Award). Some Institutes also run K12 institutional career development programs, which — like T32 — are awarded to an institution that then selects and mentors individual scholars. Degree-granting clinical research training programs

Some institutions run structured, degree-granting programs specifically for clinicians already in fellowship or early-faculty positions who need formal methodological training alongside their clinical work. The longest-running example is the NIH-Duke Master’s Program in Clinical Research, established in 1998 as a partnership between Duke University School of Medicine and the NIH Clinical Center: NIH clinical fellows, staff clinicians, and other health professionals take research-design, biostatistics, and research-management coursework (now delivered via distance learning) alongside a mentored research project, leading to a Master of Health Sciences in Clinical Research from Duke. Comparable clinical research master’s/certificate programs — sometimes still branded “Clinical Research Training Program” — exist at other academic medical centers, typically built around a similar core curriculum: study design, biostatistics, regulatory and ethical principles, and a mentored capstone project.

Certification-track training: ACRP and SOCRA

A second, distinct category of “clinical research training” has nothing to do with academic fellowships or NIH funding: workforce certification for the staff who operationally run trials — clinical research coordinators (CRCs), clinical research associates (CRAs), and related roles. The two principal US certifying bodies are the Association of Clinical Research Professionals (ACRP), which offers CRC and CRA credentials (CCRC, CCRA) among others, and the Society of Clinical Research Associates (SOCRA), which offers the Certified Clinical Research Professional (CCRP) credential. ACRP and SOCRA jointly developed a Harmonized Core Competency Framework that defines the shared competencies expected of clinical research professionals across roles and career stages, and their certification exams and continuing-education requirements are built around it. Unlike a T32/TL1/K-award fellowship, ACRP/SOCRA certification does not require NIH sponsorship, a doctoral degree, or a mentored research project — it is a professional credentialing pathway aimed at operational competency, most often pursued by CRCs, CRAs, and other clinical trial staff rather than physician-investigators.
