Yes — but “peer reviewed” means two different things when applied to NIH-funded research, and conflating them is the single most common source of confusion in this area. How NIH grant application peer review actually works
NIH uses what it calls a two-tier (or “dual”) peer review system, set out in the NIH Grants Policy Statement. In the first tier, a competing application is assigned to a study section — formally a Scientific Review Group (SRG) — where a panel of outside scientists with relevant expertise scores the application on scientific and technical merit (significance, approach, innovation, investigator qualifications, and research environment) and produces a written summary statement. Most investigator-initiated applications (R01, R21, R03, and similar mechanisms) go to a chartered, standing study section with a mix of permanent and temporary reviewer members; applications tied to specific funding opportunities, SBIR/STTR submissions, fellowships, or situations requiring specialized or conflict-free expertise instead go to a Special Emphasis Panel (SEP) convened for that one meeting.
In the second tier, the relevant Institute or Center’s national advisory council or board conducts a separate programmatic review, weighing the study section’s score and summary statement against the Institute’s funding priorities and available budget. Only after both tiers are complete does the Institute make a funding decision and, if funded, issue a Notice of Award. How journal peer review works, and why it’s a separate question
Once NIH-funded research is actually conducted and the investigators write it up, they submit the manuscript to a journal of their choosing — NIH does not select the journal, does not review the manuscript itself, and is not a party to the journal’s editorial process. The journal’s editor typically sends the manuscript to independent subject-matter reviewers (commonly two or three) who assess the work’s methodology, validity, and contribution before the editor decides to accept, reject, or request revisions. Why the two get conflated
The confusion is understandable: both processes use the same word, both involve panels of outside experts evaluating scientific work, and both sit somewhere in the lifecycle of the same research project. But they evaluate different objects at different times for different purposes. Grant application peer review asks “should NIH fund this proposed work?” before the work happens. Journal peer review asks “does this finished paper reporting completed work meet this journal’s standards for publication?” after the work is done. A study section never sees the final manuscript, and a journal’s reviewers never see (and generally have no access to) the grant application or its summary statement.
It’s also worth noting that NIH funding a study is not, by itself, a guarantee the resulting paper will pass journal peer review or be published at all — some funded projects produce results that are never submitted, are rejected, or are published only after substantial revision. NIH’s role ends with the funding decision (and post-award oversight of how the funds are used); it is not the gatekeeper for whether the resulting science is ultimately published.
