Can the same manuscript be posted to both bioRxiv and medRxiv?+
No -- a given manuscript belongs on one or the other based on its content. Basic/preclinical life-sciences work goes to bioRxiv; clinical and human-health research goes to medRxiv. Both servers' screening teams will redirect a manuscript submitted to the wrong one rather than simply rejecting it.
Why does medRxiv exclude case reports?+
Case reports carry a higher risk of being misread as clinical guidance from a single, unreplicated observation. medRxiv's screening standard specifically weighs public-harm risk, which bioRxiv's broader life-sciences scope does not need to weigh the same way.
Are bioRxiv and medRxiv still run by Cold Spring Harbor Laboratory?+
Not directly as of March 11, 2025. Both are now operated by openRxiv, an independent non-profit created specifically to run them; CSHL and, for medRxiv, Yale and BMJ remain part of the platforms' history and governance lineage.
Does posting on medRxiv prevent later journal publication?+
No. Most journals that accept preprints treat medRxiv postings the same as bioRxiv postings -- posting first does not preclude submitting to a journal afterward, and the preprint record links forward to the published version once one exists.
