Structured Abstract Format: Subheadings Across ICMJE, CONSORT, and PRISMA

A comparison of structured-abstract subheading conventions across ICMJE’s general baseline, CONSORT for Abstracts (randomized trials), PRISMA for Abstracts (systematic reviews), and AMA/APA style differences.

Neither answers a question that comes up as soon as an author’s paper falls outside plain original-research biomedical reporting: which subheadings apply, and do they change depending on what kind of study the abstract is summarizing? They do. “Structured abstract” is not one fixed template — it is a family of related formats, and the specific labeled fields a journal or reporting guideline expects differ by study type and by publisher. This guide compares the major ones side by side.

The baseline: ICMJE’s general structured-abstract requirement

The ICMJE Recommendations require original research, systematic reviews, and meta-analyses to use a structured abstract stating the study’s context or background, purpose, basic procedures, main findings, and principal conclusions — but ICMJE deliberately does not fix the exact subheadings or word count, leaving that to the individual journal. In practice, the great majority of general biomedical and life-science journals implement this as some variant of four labeled sections: Background (or Objective/Purpose), Methods, Results, and Conclusions. The two reporting-guideline extensions below build on top of this baseline for two specific, common study types: randomized trials and systematic reviews.

CONSORT for Abstracts: reporting a randomized trial

The CONSORT Statement governs how randomized controlled trials should be reported in full, and its companion extension — developed by Hopewell, Clarke, Moher, Schulz, and colleagues on behalf of the CONSORT Group, published in The Lancet and PLoS Medicine in 2008 — extends that same reporting-transparency logic specifically to the trial’s journal or conference abstract. Rather than the generic Background/Methods/Results/Conclusions labels, CONSORT for Abstracts asks authors to make sure the abstract itself — often the only part of a trial report many readers, including clinicians making point-of-care decisions, ever see — states the essential trial-design information a reader needs to judge the evidence: the trial design (e.g. parallel-group, cluster, non-inferiority), how participants were selected and how many were randomized, what the interventions and comparator were, the primary objective or hypothesis, how randomization and blinding were done, the numbers analyzed for the primary outcome and the result itself (with a measure of precision), any important harms, and — critically — the trial registration number and primary funding source. The checklist does not require a fixed set of subheadings the way ICMJE’s general format does; it requires that this specific information be present somewhere in the abstract, structured or not, and many journals implement it inside their own structured template by adding fields such as Trial Design, Randomization, Blinding, and Trial Registration alongside the usual Background/Methods/Results/Conclusions labels.

PRISMA for Abstracts: reporting a systematic review

Systematic reviews and meta-analyses have their own extension, described on the PRISMA statement’s abstracts page and originally published in PLoS Medicine (Beller et al., 2013, updated alongside PRISMA 2020). It is a 12-item checklist covering: (1) identifying the report as a systematic review or meta-analysis in the title; (2) stating the objectives, including the PICOS elements (participants, interventions, comparators, outcomes, study designs) the review addresses; (3) the eligibility criteria used to select studies; (4) the information sources searched and how current the search is; (5) the risk-of-bias assessment method used; (6) the number and type of included studies and participants; (7) a synthesis of results for the main outcomes; (8) a plain-language description of the effect, not only a statistical measure; (9) the review’s limitations; (10) an interpretation of what the results mean in practice; (11) the funding source; and (12) the review’s registration details (for example, a PROSPERO number). Journals that endorse PRISMA for Abstracts commonly translate a subset of these items into structured-abstract subheadings such as Background, Objectives, Data Sources, Study Eligibility Criteria, Study Appraisal and Synthesis Methods, Results, Limitations, and Conclusions — a noticeably more elaborate set than the plain four-part ICMJE baseline, reflecting the extra methodological transparency a review’s own search and appraisal process needs. Style-manual differences layered on top

Independently of which reporting-guideline extension applies, the house style manual a journal follows shapes formatting details within whichever subheadings are used — heading capitalization, number formatting, abbreviation rules, and reference style. The AMA Manual of Style is widely used across biomedical journals and generally pairs comfortably with ICMJE- and CONSORT-style structured abstracts. The APA Style guidelines (via APA’s Journal Article Reporting Standards, JARS) are dominant in psychology and much of the social sciences, and APA-style structured abstracts more often use headings suited to that literature, such as Objective, Method, Results, and Conclusions, sometimes adding Trial Registration for a reported clinical or behavioral trial. Neither style manual invents a new reporting-content requirement on top of ICMJE, CONSORT, or PRISMA — they govern presentation, not what information the abstract has to contain. Where a structured abstract is not the expected format

Structured format is a biomedical- and clinical-research-literature norm more than a universal one. Many engineering and computer-science venues — IEEE and ACM conference proceedings, for instance — conventionally expect a short, single-paragraph, unstructured abstract with no labeled subheadings, closer in form to PLOS ONE’s unstructured requirement (documented on CASRAI’s Abstract entry) than to ICMJE’s structured template. A conference abstract submitted for presentation selection also frequently uses a lighter, unstructured format even in fields where the eventual journal article would require a structured one. Always confirm the specific venue’s instructions for authors rather than assuming a discipline-wide default.

How to pick the right format for your submission

  • Start with the target journal’s own instructions for authors. They override every general guideline described here, including ICMJE’s own baseline.
  • Check whether your study type has its own reporting extension. A randomized trial or a systematic review should be checked against CONSORT for Abstracts or PRISMA for Abstracts respectively, even if the journal’s general instructions only mention a generic structured format — many editors expect the extension’s content items to be satisfied inside whatever subheadings the journal uses.
  • Don’t assume structure carries over between venues. A conference abstract and the eventual full journal article for the same study may use different formats entirely.

Frequently asked questions

Does CONSORT for Abstracts replace ICMJE’s structured-abstract requirement for trials?

No — it supplements it. ICMJE sets the general expectation that original research use a structured abstract; CONSORT for Abstracts specifies which trial-design content items (randomization, blinding, trial registration, harms, and so on) need to appear within that structure specifically for a randomized trial.

Is a systematic review abstract required to address all 12 PRISMA-for-Abstracts items?

Journals that endorse PRISMA 2020 and its abstract checklist expect authors to address each item, though the exact labeled subheadings used to do so vary by journal, the same way ICMJE’s general checklist content gets mapped onto journal-specific labels.

Do AMA and APA style require different structured-abstract content?

Generally no — they govern formatting and presentation (headings, number and abbreviation style, references) rather than adding or removing required content beyond what ICMJE, CONSORT, or PRISMA already specify for that study type.

My paper is not a trial or a systematic review — which format applies?

Use the journal’s own general structured-abstract instructions, typically the ICMJE-style Background/Methods/Results/Conclusions pattern or the journal’s stated variant of it; the CONSORT and PRISMA extensions only apply when the underlying study is, respectively, a randomized trial or a systematic review/meta-analysis.
