If you need a doctor in Mayagüez at night, the two verified 24-hour ERs are Mayagüez Medical Center and Hospital Perea, and only Perea advertises 24/7 pediatric care. For a common problem, Dr. Diana Nieves Castro, MD, can see you by video for US$79 while you are physically in Puerto Rico. The Ave Hostos pharmacy counters close at 10 p.m.; the nearest 24-hour counter is in Ponce.

Is this an emergency?

This is not an emergency service. Call 911 or go to the nearest emergency room if you have:

Emergency rooms in Mayagüez: Mayagüez Medical Center and Hospital Perea, 24/7

In an emergency call 911. Both of these ERs are open 24 hours per each hospital's official page. Neither is affiliated with us.

  • Mayagüez Medical Center (Dr. Ramón Emeterio Betances), Route 2, Bo. Sábalos (Ave Hostos #410). Phone 787-652-9200; adult ER ext. 72809, 72813, 72334, 78423 or 72746. Open "lunes a domingo, 24 horas" (every day, 24 hours). Unconfirmed: whether it has a pediatric ER.
  • Hospital Perea (Metro Pavia), Calle Dr. Basora #15. 24/7 ER for adults and children, with 24/7 pediatric consultations. Phone unconfirmed: its website gives 787-832-0101, federal records give 787-834-0101. If one does not answer, try the other.
  • Hospital Bella Vista (Adventist), Route 349 Km 2.7, Cerro Las Mesas. Phone 787-834-6000. Federal hospital data (CMS) lists an ER; its hours are unconfirmed. We do not link its website because it carries material unrelated to the hospital.

What happened to Hospital San Antonio?

In April 2025, Mayagüez Medical Center took over Hospital San Antonio on a 35-year contract, according to Primera Hora. Since June 2026 the hospital, now MMC San Antonio (18 Calle Dr. Ramón E. Betances N, 787-834-0050), has been under renovation, according to El Nuevo Día.

Unconfirmed: whether its ER is operating during the work, so it is not among the 24-hour ERs above. If you plan to go there, call first.

Urgent care in Mayagüez: Salus Plaza Sultana, until 9 p.m.

Salus Urgent Care Mayagüez, at Plaza Sultana, 787-792-5389, is open daily 7 a.m.–9 p.m. It is not a hospital ER and it is not open overnight. It suits a cut that may need stitches or a sprain that needs an exam. After 9 p.m., or for anything serious, go to Mayagüez Medical Center or Perea, or call 911. Is there a 24-hour pharmacy in Mayagüez? No: counters close at 10 p.m.

The Ave Hostos pharmacy counters close later than any others on the west coast, but none stays open overnight. The nearest 24-hour counter is in Ponce. From the Walgreens locator (daytime data, checked 2026-09-30) and the Farmacias Caridad directory:

  • Walgreens, 2097 Ave Hostos, 787-805-4805, and Walgreens, 992 Ave Hostos, 787-831-9251: the stores are open 24 hours; the pharmacy counters run 8 a.m.–10 p.m.
  • Walgreens, 505 Ave Hostos, 787-831-0674: pharmacy 9 a.m.–7 p.m.
  • Farmacia Caridad Medical Emporium, Route 2 Km 156.7, 787-805-4000: pharmacy Mon–Fri 7 a.m.–8 p.m., Sat 9 a.m.–6 p.m., Sun 11 a.m.–7 p.m.
  • Nearest 24-hour pharmacy counter: Walgreens, 2979 Ave Emilio Fagot, Ponce, 787-841-7400, 36.3 miles per the locator (not a drive time). Closed 1:30–2 a.m. for a meal break.

For an evening prescription, reach Ave Hostos before 10 p.m.; on a Sunday, Caridad Medical Emporium's counter is open until 7 p.m. Call first; hours change.

UPRM students: the under-21 rule

If you study at the University of Puerto Rico at Mayagüez (UPRM) and are in Puerto Rico, you can use a video visit for a sore throat, UTI symptoms, allergies, or a non-controlled prescription that ran out away from home. Note that the age of majority in Puerto Rico is 21: if you are younger, a parent with patria potestad (parental authority) or a legal guardian signs the consent, stays on camera for the whole visit and shows photo ID.

Mayagüez is also an older city: 27.5% of residents are 65 or over, and it lost 5.5% of its population between 2020 and 2024 (73,077 to 69,044), per the Census Bureau. The doctor can follow up stable high blood pressure or diabetes by video; see chronic care.

Dengue in Mayagüez and the west

According to the Puerto Rico Department of Health's weekly report for week 37 (published September 23, 2026), Mayagüez has 4 dengue cases so far in 2026 (5.8 per 100,000 residents) and the whole Mayagüez health region 8, out of 922 island-wide.
