Work and school medical certificate in Puerto Rico, by video

We complete your work and school medical certificate in Puerto Rico after a video visit: the health form from your job, school or university, plus the lab order for the certificado de salud (health certificate). What a work and school medical certificate in Puerto Rico is

It is the form or letter a doctor signs for your employer, school or university: to start a job, a practicum or an internship, to enroll, or to join a program. With this visit you don't need to take a day off to sit in an office: the doctor sees you by video, in English or Spanish.

You see Dr. Diana Nieves Castro, MD, a general practice physician licensed in Puerto Rico with several years of emergency-department experience. We see adults and children of all ages; for patients under 21, a parent with patria potestad or a legal guardian stays on camera for the whole visit and shows photo ID.

  • The lab tests, at the lab you choose.

  • Forms that need a vision exam, such as the DTOP driver's license form.

How we complete your certificate by video

Have the form ready before the visit, on paper or on your phone. The doctor goes over your history, medicines, allergies and what the form asks, and observes you on camera. She completes and signs the parts a video evaluation supports. If part of the form needs another step, such as a lab test, she explains it and, if needed, gives you a lab order for a lab near you.

Price

The forms visit costs US$35, the same for every age. You pay by card when you request the visit, and if it can't take place you get a full refund. There is no membership. See all three visit types on pricing.

If a new health problem comes up that needs a full visit, the doctor explains why and, with your consent, you pay the difference. You pay the lab directly for any lab tests.

Medical certificate for work in Puerto Rico

If your employer asks for a medical certificate or form before you start a job, a practicum or an internship, bring it to the visit. The doctor completes what her video evaluation supports.

If you will work where food is handled or where direct health services are given to the public, you will probably also be asked for the certificado de salud. That is a separate document with lab tests, explained below.

Medical certificate for school and university

Common forms the doctor can complete after evaluating you:

  • School: health forms for enrollment or school activities. School requirements are covered in our guide to Puerto Rico school health requirements.
  • University: health forms for admission, student housing, a practicum or a program.

The health certificate lab order

The certificado de salud is governed by Law 232 of 2000 and the Department of Health's Regulation 138. It is required every year for employees of places that handle food or give direct health services to the public. It requires a medical evaluation, a TB test from the last 3 months (a TB skin test or a TB blood test) and a syphilis test from the last 6 months. It is valid for one year.

In the forms visit we give you the lab order for the blood tests. We do not issue the certificate: a doctor authorized by the Department of Health issues it, only at their authorized site. If you use the TB skin test, the authorized doctor or their nursing staff places it, and it is read in person 48 to 72 hours later. What this visit doesn't cover

We don't complete forms that need a vision exam or another exam that can only be done in person. For example, the driver's license medical form (DTOP) needs a vision exam: our driver's license medical certificate guide explains why.

When to go to the ER

This visit is for paperwork. If you have warning signs such as chest pain or trouble breathing, call 911 or go to the emergency room.

  • If appropriate, the doctor prescribes and we send your prescription to your pharmacy. We do not prescribe controlled substances.

  • You must be physically in Puerto Rico during the visit (visitors, tourists and cruise passengers are welcome).

  • In an emergency, call 911.

Frequently asked questions

How do I get a medical certificate for work in Puerto Rico?

Ask your employer for the form or letter they need, book a forms visit (School and work forms, US$35), and have the form and your photo ID ready. The doctor evaluates you by video and signs what the evaluation supports. If your job also needs the certificado de salud, we give you its lab order in the same visit.

Will the doctor sign any certificate?

She signs what her video evaluation supports. If part of the form needs another step, such as a lab test, she explains it. We can't guarantee a signature before she evaluates you.

Will my employer or school accept a certificate done by video?

Each employer, school or university decides what it accepts. Before you book, ask whether they accept a form completed after a telehealth evaluation and whether the form asks for any in-person exam.

Is a medical certificate the same as the certificado de salud?

No. A work and school medical certificate is the form your employer or school asks for. The certificado de salud is the yearly Department of Health document with TB and syphilis tests, issued by an authorized doctor. For that one, we give you the lab order.

Do you complete the driver's license form?
