This page is for San Juan's residential barrios. Hato Rey had 41,881 residents and Cupey 32,833 at the 2020 Census; the whole municipio had 332,454 in 2024, and 26.8% are 65 or older. Plenty of households here get around without a car for every errand: the Tren Urbano stops at Hato Rey (the Golden Mile), Universidad (UPR Río Piedras), Río Piedras, Cupey and Centro Médico, the station for the Centro Médico complex and the UPR Medical Sciences Campus.

  • Adults 24/7 plus a pediatric ER: Hospital San Francisco, 371 Calle José de Diego, Río Piedras, PR 00923. Phone 787-767-5100. On Metro Pavia's 24/7 adult ER list and on its pediatric ER list; the pediatric list does not publish hours.
  • Adults 24/7 plus a pediatric ER: Hospital Metropolitano, Carr. 21 #1785, Las Lomas, San Juan, PR 00921, the ER for Puerto Nuevo and Caparra Heights. Phone 787-782-9999.
  • Adults 24/7: Hospital Episcopal San Lucas Metro, Carr. 844 Km 0.5, Cupey, PR 00928, on the former HIMA Cupey site. Phone 787-761-8383. Its site advertises a 24/7 ER but does not say whether it sees children.
  • Adults and children: Hospital Auxilio Mutuo, Ave. Ponce de León, Stop 37½, Hato Rey. Phone 787-758-2000; adult ER ext. 2656 or 2696, pediatric ER ext. 1312 or 1426. Its directory does not publish hours.
  • Children only: Hospital Pavía Hato Rey, 435 Ave. Ponce de León, Hato Rey, PR 00917. Pediatric ER 787-641-2323. It is not on Metro Pavia's adult ER list; we could not confirm an adult ER.
  • Chest pain: Puerto Rico and Caribbean Cardiovascular Center, Ave. Américo Miranda, Centro Médico. Phone 787-754-8500, ext. 1122, 1124, 1143 or 1134. A specialized 24/7 ER for heart emergencies only.
  • Major trauma: Centro Médico (ASEM), Bo. Monacillos, 787-777-3535. The island's highest-level trauma center.
  • Veterans: San Juan VA Medical Center, 10 Calle Casia, San Juan, PR 00921, 800-449-8729. Emergency department open 24/7, for veterans.
  • ER per CMS, hours not published: Hospital Pediátrico Universitario (787-777-3232) and Hospital Universitario de Adultos (787-754-0101), both at Centro Médico, and Hospital del Maestro, 550 Calle Sergio Cuevas Bustamante, Hato Rey (787-758-8383). Call first.

The Municipal Hospital, Hospital del Maestro and Centro Médico: what to know

Federal data still shows an ER at the Hospital Municipal de San Juan in Monacillos, but El Nuevo Día reported in 2017 that it closed after Hurricane Irma, and we found no report of it reopening, so we leave it off. Primera Hora reported in 2025 that Hospital del Maestro owed US$11.8 million for electricity and US$870,000 for water; CMS still lists its ER, but phone first.

Centro Médico is for a serious crash, a heart attack or a critical case. It is not for a UTI: in 2018 one patient waited 24 hours in its ER (El Nuevo Día), and in 2023 it opened US$4 million of new waiting rooms to end corridor waits (Primera Hora).

24-hour pharmacy in San Juan: the Hato Rey counter closes at 11 p.m.

Only five pharmacy counters in Puerto Rico are open 24 hours, all Walgreens, and none is inside the San Juan municipio. Store hours and pharmacy-counter hours are different things:

  • Walgreens, 685 Calle César González, Hato Rey, PR 00918, 787-294-1730. Store: 24 hours. Pharmacy counter: daily 7 a.m.–11 p.m., closed 1:30–2 p.m. The latest counter we verified in these neighborhoods.
  • Farmacia Caridad Ave. Alejandrino, 310 Ave. Lomas Verdes, San Juan, PR 00921, 787-740-7000. Pharmacy counter: Mon–Fri 7 a.m.–10 p.m., Sat 8 a.m.–9 p.m., Sun 9 a.m.–8 p.m.; it lists a drive-thru.
  • Farmacia Caridad Hato Rey, 374 Ave. Ponce de León, 787-766-7200. Store: until 9 p.m. Pharmacy counter: Mon–Fri 7 a.m.–7 p.m., Sat 9 a.m.–5 p.m., Sun 11 a.m.–5 p.m.
  • Nearest 24-hour pharmacy counter: Walgreens, 2505 Carr 841, Guaynabo, PR 00969, 787-287-0528, 3.6 miles from Río Piedras (ZIP 00918) per the Walgreens locator. Closed 1:30–2 a.m. for a meal break.

Hours change, so call the counter before heading out at night.

Is there urgent care open at night in Río Piedras, Hato Rey or Cupey?

We could not verify, from a primary source, any urgent care center or CDT in Río Piedras, Hato Rey or Cupey with published hours, so we list none; the municipio runs CDTs, but we could not confirm which are open or when. Doctors' Center Barrazas in Carolina is an urgent care with verified hours, and it closes at 9 p.m. At night, the verified options are the hospital ERs above. Sick at night in Río Piedras, Hato Rey or Cupey

At night the gap in these neighborhoods is usually an open pharmacy counter, not an ER. Work through it in this order:

  1. Is it an emergency? Chest pain, shortness of breath, facial droop or a bad crash: call 911. San Francisco, Metropolitano and San Lucas Metro see adults 24 hours.
  2. A very sick child? San Francisco, Auxilio Mutuo and Pavía Hato Rey have pediatric ERs; call ahead if you can. For minor problems, see video visits for children.
  3. Need a prescription filled before 11 p.m.? The Walgreens counter on Calle César González is open until then every day.
  4. After 11 p.m.? The nearest open counter is Walgreens, 2505 Carr 841, Guaynabo, closed 1:30–2 a.m.

Dengue in San Juan: 41 cases in 2026

The Puerto Rico Department of Health's weekly report for week 37 (published September 23, 2026) counts 41 dengue cases in the San Juan municipio in weeks 1–37, about 12.3 per 100,000 residents, and 97 in the San Juan health region. One of the island's four dengue-associated deaths this year was in that region: a 90-year-old woman, in February. Which ER near Río Piedras sees children?
Hospital San Francisco on Calle José de Diego in Río Piedras (787-767-5100) has a 24/7 adult ER and is on Metro Pavia's pediatric ER list. In Hato Rey, Auxilio Mutuo and Pavía Hato Rey also have pediatric ERs. If a child is struggling to breathe or unresponsive, call 911.
Can a doctor come to my home in San Juan?
No. We do not make house calls; we see patients by video only. The doctor sees you by video at home in Río Piedras, Hato Rey or Cupey, as long as you are in Puerto Rico. If you need a hands-on exam, use one of the ERs on this page or your regular doctor.
How much is a doctor visit without insurance in San Juan?
Each provider sets its own price. Our video visit is US$79, charged when you request it, with an itemized receipt (superbill) on request.
