See a Doctor in Toa Baja, Levittown and Toa Alta by Video

If you need a doctor in Toa Baja, Levittown or Toa Alta, know that neither municipio has a hospital: the nearest verified round-the-clock ER is SIM in Toa Alta town, and the hospitals are in Bayamón. If it is not an emergency, you can skip the drive: Dr. Diana Nieves Castro, MD, sees you by video for US$79 while you are in Puerto Rico. In an emergency, call 911.

Is this an emergency?

This is not an emergency service. Call 911 or go to the nearest emergency room if you have:

  • Chest pain, shortness of breath or stroke signs
  • A child who is struggling to breathe, hard to wake or cannot keep fluids down
  • Fever after heavy rain with severe belly pain or bleeding (possible severe dengue)
  • A deep or dirty wound after cleaning up flood debris

No hospital and no CDT in Toa Baja: where do you go?

Federal hospital data (CMS) lists no hospital with an ER in either Toa Baja or Toa Alta. Toa Baja also sold its CDT (the municipal diagnostic and treatment center) in 2016 to pay down municipal debt, according to Primera Hora. We could not verify any ER in Levittown.

That leaves three options: SIM's ER in Toa Alta, the hospitals in Bayamón, or a video visit when it is not an emergency. Not sure which? Read where to go when you are sick.

Toa Alta's 24/7 ER (SIM) and urgent care at CSI Toa Alta II

Salud Integral en la Montaña (SIM) runs the former Toa Alta CDT. It is the only CDT-style ER open around the clock that we could verify in the metro suburbs. It is not affiliated with us.

  • SIM Emergency Room, 16 Calle Antonio R. Barceló, Toa Alta. Phone 787-870-3322. Open 24 hours, 7 days, for adults and children, per its official page.
  • SIM CSI Toa Alta II, Carr. 165 Km 4.5, Bo. Quebrada Cruz. Phone 787-545-8808. Extended hours: Mon–Fri 8 a.m.–9 p.m., Sat 8 a.m.–6 p.m. It is not a 24-hour ER: at night, go to SIM's ER in town or call 911.

Nearest hospitals: Bayamón

  • Bayamón Medical Center, Carr. 2 Km 11.7, Bayamón. Phone 787-620-8181. Adult ER open "24/7 365 días".
  • Puerto Rico Children's Hospital, Carr. PR-2 Km 11.6, Bayamón. Phone 787-474-8282. A children's hospital; registration is through its ER, open 24/7.

Levittown pharmacies: a 24-hour store is not a 24-hour counter

The Walgreens on Ave Los Dominicos keeps its store open all night, but not its pharmacy counter. From the Walgreens locator and the Farmacias Caridad directory:

  • Walgreens, 35 Ave Los Dominicos, Levittown, 787-795-2083: store open 24 hours; pharmacy opens 7 a.m.
  • Walgreens, 100 Calle Colina Real, Toa Baja, 787-779-8385: pharmacy opens 9 a.m.
  • Walgreens, Plaza Aquarium, Carr. 165 Km 4.7, Toa Alta, 787-870-4776: pharmacy opens 8 a.m.
  • Farmacia Caridad Levittown, 60 Ave Los Dominicos, 787-795-8630: pharmacy Mon–Fri 7 a.m.–9 p.m., Sat 8 a.m.–8 p.m., and Sun 10 a.m.–8 p.m.

None of the island’s five 24-hour pharmacy counters (all Walgreens) is in Toa Baja or Toa Alta. The nearest is Walgreens, 4699 Route 5, Bayamón, 787-799-4699, closed 1:30–2 a.m. for a meal break. Another is on PR-841 in Guaynabo. Call before you go; hours change.

Floods and the Río La Plata: prepare your medications

During Hurricane María the Río La Plata rose more than 11 feet on September 20, 2017, and 2019 flood maps describe Toa Baja as extremely vulnerable to flooding. The area around Lago La Plata in Levittown flooded too.

If you live in a flood zone, keep a list of your medications and doses, photos of the labels and a spare supply in a waterproof bag you can grab. If floodwater takes your non-controlled medications, the doctor can assess by video whether it is appropriate to prescribe replacements. See medications and hurricanes.

Dengue in Toa Baja and Toa Alta

According to the Puerto Rico Department of Health's weekly report for week 37 (published September 23, 2026), Toa Baja has 27 dengue cases so far in 2026 (37.9 per 100,000 residents) and Toa Alta 17 (25.7 per 100,000). A doctor in Toa Baja by video: telehealth for young families

Toa Alta has the youngest population of the metro areas we cover: only 18.5% are 65 or older, per the Census Bureau (Toa Baja: 23.3%). Levittown, with 25,591 residents in the 2020 Census, is a large family community, and with no hospital in town, a child's fever or cold at night often means a drive to Bayamón or to SIM's ER.

We see children from 6 months, with a parent with patria potestad (parental authority) or a legal guardian on camera for the whole visit; see video visits for children. We could not verify one. The nearest verified 24-hour ER is SIM in the former Toa Alta CDT (787-870-3322), and the nearest hospitals are in Bayamón. In an emergency call 911.
Does the SIM ER in Toa Alta see children?
Yes. Per its official page, SIM's ER on Calle Antonio R. Barceló is open 24 hours for adults and children. For a very sick child there is also Puerto Rico Children's Hospital in Bayamón.
What happened to the Toa Baja CDT?
The municipality sold it in 2016 to pay debt, according to Primera Hora. There is no verified ER inside Toa Baja today.
Is there a 24-hour pharmacy in Levittown?
The Walgreens store on Ave Los Dominicos is open 24 hours, but its pharmacy counter opens at 7 a.m. The nearest 24-hour counter is Walgreens, 4699 Route 5, Bayamón. Caridad Levittown's pharmacy stays open until 9 p.m. on weekdays.
My house flooded and I lost my medications. Can you help?
If they are not controlled substances, the doctor can assess by video whether it is appropriate to prescribe them. Have the names and doses ready. If you feel unwell or have run out of an essential medication such as insulin, go to an ER.
Is CSI Toa Alta II urgent care open at night?
No. CSI Toa Alta II on PR-165 is open Mon–Fri 8 a.m.–9 p.m. and Sat 8 a.m.–6 p.m. At night, the verified option is SIM's 24/7 ER in Toa Alta town, or 911.
