Couples wellness retreat in Puerto Rico: coming together, and what the week is
A couples wellness retreat is one that two people can attend together. THI’s retreat is open to everyone, so couples of any kind are welcome, and so are people coming alone or with friends. Each person applies individually, pays the per-person price and has their own physician assessment. It is not relationship counseling or a romantic package, and we make no promises about your relationship. The retreat is held in English.
Is it an emergency? Call 911.
Pay now, then choose your time right away. Full refund if you cancel 24 hours or more ahead.

What this is, and is not
The Health Initiative (THI) runs an in-person wellness retreat in Puerto Rico: 5 days starting November 15, 2026. It is open to everyone. Couples of any kind can come, and so can people traveling alone, with friends or with family. There is no separate couples program: it is the same retreat, and you do it together.
What it is not: it is not relationship counseling, it is not marriage or sex therapy, and it is not a honeymoon package or a romantic getaway. We make no promises about your relationship. It is a week to rest, move, learn about habits and have a physician assessment, shared with your partner if you like.
Applying together to a couples wellness retreat
Each person fills in their own application. On the application, tell us you are applying together. We can’t promise in advance that both of you are accepted, that two places are available, or that you share a room.
The cohort has 10 places and applications close November 5, 2026. The price is US$4,500 per person, so each of you pays your own. There is no couples rate or discount. If your application is accepted, you reserve your place with a deposit.
A deposit reserves your place. The amount and terms are confirmed in your acceptance email.
Rooms and time together
Simple, comfortable lodging at the hacienda. Room details, including whether rooms are shared or private, are confirmed in your acceptance email.
A typical day, not a fixed schedule (we share the full schedule with accepted guests): gentle morning movement such as gentle stretching and mobility, breakfast together, a medical or education session, free time or an island activity, dinner, and an evening meditation, with phones kept away during sessions and meals.
If you want to know which parts of the week are optional and how much free time there is, ask us before you apply. What you do with that time is up to you and your partner.
Two separate medical assessments
Each person has their own assessment with Dra. Diana Nieves Castro, a general practice physician licensed in Puerto Rico. What the physician discusses and decides with each of you is individual.
A physician assessment with Dra. Diana Nieves Castro, MD, and lab work when it is clinically appropriate. She provides all the medical care at the retreat.
Labs are ordered only when clinically appropriate for that person. One partner’s health situation does not decide the other’s plan. Each of you brings your own medications; see how to travel with medications.
Doing it with different energy levels
It is common for one person to want to do more than the other. If that is you, ask us before you apply how the week can be adapted.
Examples of what the area offers and what the days may include: a walk in El Yunque rainforest, time at Luquillo beach, a gentle stretching session, and relaxed evenings outdoors. The activities for your cohort are confirmed with accepted guests.
Tell us about your mobility, dietary or other access needs on the application, and we will work with you on what is possible.
The CDC recommends 150 minutes of moderate activity a week for adults and says some physical activity is better than none. That applies to each of you separately.
Talk to us first if
Contact us before you apply if you are pregnant, had surgery recently, have a condition that isn’t stable, were recently hospitalized or have mobility needs. This is about safety: it lets us tell you plainly what is possible. It applies to each partner separately.
If there is violence or fear in your relationship, don’t wait for a retreat: call the National Domestic Violence Hotline at 1-800-799-7233, or 911 if you are in immediate danger. In a mental health crisis, call or text 988 or call Línea PAS at 1-800-981-0023.
If your partner won’t attend
You can come without your partner: each person applies individually. If something specific worries you, such as how the week would work or what happens with the room, ask us before you apply and we will tell you what we know.
To plan the trip and the full cost, see what a wellness retreat includes and costs and what to expect at a wellness retreat. If you are curious about meditation or breathing, see the guides to breathwork and guided meditation.
Other ways to come
The retreat is the same for everyone. If another way of coming suits you better, see the solo wellness retreat or the wellness retreat with friends or family.
How we care for you
We evaluate, diagnose and treat; if needed, we give you a lab order for a lab near you.
We see adults and children of all ages. Patients under 21: a parent with patria potestad (parental authority) or a legal guardian is on camera for the whole visit and shows photo ID.
Real-time video visit. We send you a secure video link. It works on your phone, tablet or computer.
If appropriate, the doctor prescribes and we send your prescription to your pharmacy.
You must be physically in Puerto Rico during the visit (visitors, tourists and cruise passengers are welcome).
In an emergency, call 911.
