Affordable wellness retreats: what they include, what they cost and how to compare

An affordable wellness retreat is one whose price already covers what you need: lodging, meals, the sessions and any medical care, with no mandatory fees added later. Cost depends mostly on length, group size, lodging, staff and location. Before you pay, compare the price per day, exactly what is included in writing, who provides any medical care, and the cancellation terms.

What is a wellness retreat?

A wellness retreat is a planned stay of a few days with a small group and a program built around rest, movement, food and habits. Unlike a vacation or a spa, where you pick each service on your own, a retreat runs a set program led by a team.

A retreat center is a property set up to host groups: rooms, a kitchen, session spaces and often outdoor areas. Some organizers own their center; many rent one for each group. A medical and wellness retreat also adds a licensed health professional who assesses each guest.

What is included in a wellness retreat, and what costs extra

Every retreat packages things differently. In general, this is what is usually in the price and what is often charged on top:

  • Usually included: lodging, meals, the program sessions (movement, meditation, talks), some activities and sometimes airport transfers.
  • Often extra: airfare, tips, lodging taxes and fees, a single-room supplement, optional treatments such as massage, extra excursions and travel insurance.

Ask for the list of inclusions in writing before you pay. If a page doesn’t spell it out, ask.

How much does a wellness retreat cost?

There is no standard price. What moves the price of a wellness retreat most:

  • Length (which is why the price per day is the fairer comparison)
  • Group size: small groups cost more per person, but you get more attention
  • The lodging, and whether rooms are shared or private
  • How many staff there are and how they are trained
  • Whether a licensed clinician provides medical care and labs
  • Location and season

All-inclusive wellness retreats: what it should mean

“All-inclusive” should mean the price you see already contains every mandatory fee. The Federal Trade Commission’s Rule on Unfair or Deceptive Fees, in effect since May 12, 2025, requires hotels, vacation rentals and other short-term lodging to show the total price including all mandatory fees. A retreat package is not always covered by that rule, but it is a good standard to ask for.

Even an all-inclusive retreat almost never includes airfare. Also ask about tips, taxes and the cost of any optional services.

How to compare retreats and find an affordable wellness retreat

Before you pay, check:

  • Price per day: divide the total price by the number of program days.
  • Group size, and how many staff look after the group.
  • Exactly what is included, in writing: lodging, meals, sessions, activities and transfers.
  • Who provides any medical care, and whether they are licensed. You can verify a doctor’s license in Puerto Rico.
  • Cancellation terms and what happens if weather disrupts flights, especially during hurricane season.
  • Accessibility: stairs, terrain, and meals for allergies or special diets.
  • The language the program is held in.
  • Health promises: be wary of anyone who says a few days will fix a chronic disease. There is no good evidence that detox diets or products remove toxins.

Where The Health Initiative’s retreat fits

The Health Initiative (THI), the US 501(c)(3) nonprofit that runs this clinic, offers an in-person medical and wellness retreat in Puerto Rico: 5 days starting November 15, 2026, with 10 places. The price is US$4,500 per person, and applications close November 5, 2026.

The price includes:

  • Lodging and meals
  • Medical care and labs: a physician assessment and lab work when clinically appropriate
  • Coaching and sessions: mobility coaching, meditation and breathwork, and nutrition
  • Island activities and airport transfers

Airfare is not among the inclusions. Dr. Diana Nieves Castro, a general practice physician licensed in Puerto Rico, provides all the medical care at the retreat, including labs. The approach is a root-cause one in the style of functional medicine, led by a licensed physician: assessment, education, habits and coaching.

The retreat is open to everyone (who it is for). It is held in English at a hacienda in Río Grande, Puerto Rico; the exact address is shared with confirmed guests.

Simple, comfortable lodging at the hacienda. Room details, including whether rooms are shared or private, are confirmed in your acceptance email.

Fresh, home-style meals built around whole foods, with Puerto Rican flavors. Tell us about allergies and food preferences on your application.

To join, you apply first; if you are accepted, you reserve your place with a deposit.

A deposit reserves your place. The amount and terms are confirmed in your acceptance email.

For your safety, talk to us before you apply if you are pregnant, had recent surgery or have a condition that isn’t stable. Ask us anything before you apply.

We share the deposit and the payment terms with you before you pay anything.

To see how the program works day to day, read what to expect at the retreat, or see the wellness retreat.

Are wellness retreats worth it?

A retreat can be worth it if you want a few days of rest, time to learn and practice habits, and unhurried time with a physician and coaches. A retreat is not a cure, and a few days on their own rarely change your health: what counts is what you keep doing at home.

Before you go, plan how you will keep the habits up when you get back and who will follow up with you. Ideally, take what you learned and any results to your own doctor.

If you are going through a hard time with your mental health, a retreat is not the first step: see mental health care, or call or text 988, or Línea PAS at 1-800-981-0023, any time.

What to wear and pack for a wellness retreat

Every program has its own list; for a warm climate like Puerto Rico’s, plan on:

  • Light, loose clothes you can move in, and comfortable shoes
  • Swimwear, a hat and a light layer for air conditioning or evenings
  • Sunscreen with UVA and UVB protection, SPF 15 or higher, as the CDC advises. Read about sunburn, and drink water often in the heat.
  • Insect repellent with DEET or picaridin, as the CDC recommends
  • Your medicines in your carry-on, preferably in their original bottles. See traveling to Puerto Rico with medications.
  • The Health Initiative’s own packing list is in what to expect at the retreat.

Insurance, HSA and FSA

Health insurance generally does not pay for a wellness retreat. If you have an HSA or FSA, ask your plan administrator.

If you are physically in Puerto Rico and want to talk with the doctor before you apply, you can book an online visit. If you live elsewhere, talk to your own doctor first.

How we care for you

  • We evaluate, diagnose and treat; if needed, we give you a lab order for a lab near you.

  • We see adults and children of all ages. Patients under 21: a parent with patria potestad (parental authority) or a legal guardian is on camera for the whole visit and shows photo ID.

  • Real-time video visit. We send you a secure video link. It works on your phone, tablet or computer.

  • If appropriate, the doctor prescribes and we send your prescription to your pharmacy.

  • You must be physically in Puerto Rico during the visit (visitors, tourists and cruise passengers are welcome).

  • In an emergency, call 911.

How do I know a retreat’s medical care is real?
