Winter wellness retreat in Puerto Rico: before or after the holidays
A winter wellness retreat is a short, physician-supported stay in a small group to rest before the holidays or start the new year steadier. A new group starts on the 1st and 15th of every month; the first begins November 15, 2026. The retreat is held in English. It is not treatment.

Is this an emergency?
This is not an emergency service. Call 911 or go to the nearest emergency room if you have:
- Thoughts of harming yourself or ending your life: call or text 988, or call Línea PAS at 1-800-981-0023
- Sadness, hopelessness or loss of interest most days for two weeks or more
- Chest pain, fainting or shortness of breath: call 911
- A health condition that changed recently, or a new medicine, before you travel: talk to your own doctor first

It is December, and you are already tired
The list does not get shorter. Gifts, dinners, travel, family, the year-end push at work. You go to bed late, wake up with your head already running, and tell yourself that once the holidays are over, you will rest. Then January arrives with a list of its own.
Many people wait until they are worn out to stop. So the holidays are lived on autopilot, and the new year starts with the same energy the old one ended on: not much. That is the cost of staying as you are, and it does not appear on any calendar.
You may know the scene: finally sitting on the couch at ten at night, too tired to pick a film. Your body is home, but your head is still on the list. That kind of tiredness does not yield to one longer night. It asks for a change of place and pace, and for someone to tell you that you may stop.
A winter wellness retreat is another option. You step away for a few days, in a small group, with a licensed physician looking after the clinical side, and you come back with a simple plan instead of another new year promise. It does not fix your calendar. It gives you a place where you are finally allowed to stop.
A new group starts on the 1st and the 15th of every month
The first group starts November 15, 2026 and runs 5 days, with 10 places. Applications for that first group close November 5, 2026.
After that, a new group starts on the 1st and the 15th of every month. If you are searching for a wellness retreat in December or a new year wellness retreat, that gives you four concrete start dates: December 1, December 15, January 1 and January 15. Each group is small, so each guest gets close attention.
Two things matter here. First, a group is a start date, not a promise of availability: each group is small, and your place is reserved when you pay the deposit. Second, applying early gives you room for flights and for a conversation with your own doctor if you need one. The earlier you apply, the more start dates you have to choose from.
You reserve your place on the retreat page: fill in the short form and pay the deposit through secure checkout. Our team then writes to you, and our physician reviews your health in a private step. If the retreat is not a good fit for you, we refund your deposit in full. The deposit and cancellation terms are on the retreat page and shown again at checkout. For the steps, read how to apply to the retreat.
Before the holidays or after them: how to choose
There is no correct date, only the one that fits your life. These questions help:
- If you go to a wellness retreat before the holidays (for example, in November 2026): you arrive in December with some rest behind you and one or two habits already tried. It suits you if the dinners, gifts and travel are your hardest stretch of the year.
- If you would rather wait until the holidays are over: you enter the new year with a place to reset. It suits you if December is unavoidable and what you need is a starting point for January.
- If your work slows down at year end: see whether a December or January start falls inside that pause, so you do not spend leave you would rather keep.
- If you are travelling from the mainland US: think about year-end flights, which tend to fill early. Read how to get there from US cities.
How the days work, and what to expect from the weather
The Health Initiative (THI) runs an in-person medical and wellness retreat in Puerto Rico: 5 days, 10 places in the first group, at $3,500 per person. It is held at a hacienda in Río Grande, Puerto Rico. To place the area, read about El Yunque and Rio Grande. The retreat is held in English, and it is open to everyone.
Puerto Rico is a tropical island, so pack for warm days and the occasional rain shower rather than for a northern winter. For what the season is like, read Puerto Rico in November.
Here is what a typical day looks like. It is not a promised schedule; see what to expect at the retreat for more.
A typical day, not a fixed schedule (we share the full schedule with accepted guests): gentle morning movement such as gentle stretching and mobility, breakfast together, a medical or education session, free time or an island activity, dinner, and an evening meditation, with phones kept away during sessions and meals.
Included in the price:
- Lodging and meals
- Medical care and labs when clinically appropriate, provided by Dra. Diana Nieves Castro, a general practice physician licensed in Puerto Rico (see the physician visit and labs)
- Mobility coaching, meditation and breathwork, and nutrition sessions
- Island activities and airport transfers
The honest objections: is it worth it? What if I am not a yoga person?
Is it worth it? That depends on what you expect. It does not cure anything and it does not change your job. What it does is take you out of your routine, give you time to sleep and move at regular hours, and show you a plan you can keep. The CDC recommends 7 or more hours of sleep a night for adults 18 to 60, on a regular schedule. Many people do not get a single week like that all year.
I am not spiritual, and I do not do yoga. You do not have to. Meditation and breathwork are guided sessions, and you take part at your own pace. If you would rather walk, eat well and sleep, that is the retreat too. To see whether it fits, read who the retreat is for.
Can I afford the time and the cost? Both are fair questions. The price is $3,500 per person and covers what is listed above; for the breakdown, read what a wellness retreat includes and costs. The time is short on purpose, and a few days is easier to plan than a long vacation.
Is it safe with my health condition? It depends, which is why we talk first. If you are pregnant, had recent surgery or have a condition that is not stable, talk to us before you apply. Read the retreat with chronic conditions. The retreat does not replace your own doctor.
When it is more than year-end tiredness
The holidays weigh more on some people than on others, and there is nothing weak in that. If your mood has been low most days for two weeks or more, you have lost interest in things you used to enjoy, or you are thinking about harming yourself, that needs care, not a trip. Talk to a doctor or a mental health professional soon.
In a crisis, call or text [988](tel:988) (it works in Puerto Rico and on the mainland; press 2 for Spanish), call Línea PAS at 1-800-981-0023, or call 911. If the strain is mostly work, you can also read about the burnout retreat.
How we care for you
We evaluate, diagnose and treat; if needed, we give you a lab order for a lab near you.
We see adults and children of all ages. Patients under 21: a parent with patria potestad (parental authority) or a legal guardian is on camera for the whole visit and shows photo ID.
Real-time video visit. We send you a secure video link. It works on your phone, tablet or computer.
If appropriate, the doctor prescribes and we send your prescription to your pharmacy.
You must be physically in Puerto Rico during the visit (visitors, tourists and cruise passengers are welcome).
In an emergency, call 911.
