Is this an emergency?
This is not an emergency service. Call 911 or go to the nearest emergency room if you have:
- Chest pain, shortness of breath or fainting: call 911, do not wait to travel
- A recent change in a health condition, or a new medicine, before you travel: talk to your own doctor first

The bill nobody showed you when you booked
Imagine that on another occasion you booked a few days of rest somewhere else. You arrived, and the rest came with fine print: meals were extra, the activity you wanted cost more, the airport transfer was charged per person, and the one day you wanted to talk to someone about your health, nobody knew who that was. You flew home more tired of the bill than of your life.
That is why so many people now search for an all-inclusive wellness retreat. Not because they want someone else to decide for them, but because they want to put down, for a few days, the question of what the next step will cost. A clear fee, one bill, and a clear head for what you came to do.
Here we say precisely what “all-inclusive” means at THI’s retreat in Puerto Rico. It includes what is on the list below; you book your flight to Puerto Rico separately. The retreat is held in English.
What the price covers
The retreat is $3,500 per person. That covers 5 days starting November 15, 2026. This is the full list of what that price includes:
- Lodging and meals
- A physician assessment, and labs when clinically appropriate
- Mobility coaching, meditation and breathwork, and nutrition sessions
- Island activities and airport transfers
What each line means in practice
Lodging and meals. Simple, comfortable lodging at the hacienda. Room details, including whether rooms are shared or private, are confirmed in your acceptance email. Fresh, home-style meals built around whole foods, with Puerto Rican flavors. Tell us about allergies and food preferences on your application.
Physician assessment and labs. Dra. Diana Nieves Castro, a general practice physician licensed in Puerto Rico, does the assessment and orders labs when clinically appropriate. We do not promise specific tests or results. For the detail, read the physician visit and labs.
Coaching and sessions. Sessions are led by your mobility coach, your meditation and breathwork guide, and your nutrition guide, alongside Dra. Diana Nieves Castro for the medical side. We introduce the team to accepted guests.
Island activities. They are part of the fee and they are optional; which ones happen depends on your group’s itinerary (the list of possibilities is just below). To see what kind of days these are, read the adventure wellness retreat and the nature retreat.
Transfers. Arrive on November 15, 2026; the retreat lasts 5 days and you depart after the last one. Airport transfers are included, and we confirm arrival and departure details with accepted guests.
Which activities can be part of the fee
The fee covers the activities on the itinerary, but each group has its own itinerary. That is why the list below is possibilities and not a promise: it depends on your start date, the season, the weather, sea conditions and your physician assessment.
Every activity is optional.
- Snorkeling
- Kayaking or paddleboarding on calm water
- Beach time and swimming
- A walk in El Yunque rainforest
- A visit to a waterfall or river pool, depending on conditions
- Birdwatching and wildlife walks, such as listening for coquí frogs at night
- An evening trip to a bioluminescent bay, depending on the season and the moon
- A boat trip to a cay or small island
- A gentle adventure activity, such as a zip line, for those who want it
- A horseback ride
- A visit to a coffee or cacao farm
- A Puerto Rican cooking class or a dance class
- An afternoon in Old San Juan
- An evening of stargazing
- Gentle outdoor movement in the morning
Not every activity runs in every group. We confirm your group's itinerary with accepted guests, and every activity is optional.
What you book separately
The price includes the four lines on the list above. You book your own flight to Puerto Rico and home again.
Why a clear-fee retreat is not the same as an all-inclusive resort
At a large resort, “all-inclusive” usually means a buffet, a bar and an open schedule shared with hundreds of guests. It is a good way to rest, and there is nothing wrong with it. But it usually does not include a physician who reads your history, or a small group where someone notices you have not been sleeping.
THI’s retreat is a different thing. It is a private stay for 10 places, at a hacienda in Río Grande, Puerto Rico, with a physician assessment, led sessions and meals planned for the group. You gain close attention and a rhythm that makes sense from day one. You give up the variety and the anonymity of a big resort. To compare calmly, read wellness retreat or vacation.
The exact location is shared with confirmed guests.
The honest questions
What about anything that is not on the list? The price covers the four lines above. If you are unsure about something that does not appear, write to us before you reserve. The breakdown is also in what a wellness retreat includes and costs.
How do I pay? You reserve your place on the retreat page with a deposit through secure checkout. The deposit reserves your place and is paid when you reserve, through secure checkout. The deposit and cancellation terms are on the retreat page and are shown again at checkout. The terms are on the retreat page and shown again at checkout.
What if the retreat is not right for me? If the retreat is not a good fit for you, we refund your deposit in full. You can read the full terms on the retreat page before you pay.
Is it safe with my health condition? Often yes, but talk to us first if you are pregnant, had recent surgery or have a condition that is not stable. Tell us about your mobility, dietary or other access needs on the application, and we will work with you on what is possible.
Is it treatment? No. It is a wellness stay: assessment, education, habits and coaching. It does not replace your own doctor and it does not promise outcomes.
To see whether it fits you, read who the retreat is for. It is open to everyone.
The first step
Nobody needs to decide everything today. Read what you want to read, set the included list beside any other option, and reserve your place when the numbers add up for you.
The first step is small: fill in the short form on the retreat page and pay the deposit through secure checkout. Our team then writes to you, and our physician reviews your health in a private step. The steps are in how to apply to the retreat. To see what each day holds, read what to expect at the retreat.
If you are going through a hard time with your mental health, a retreat is not the answer: call or text 988, or call Línea PAS at 1-800-981-0023.
How we care for you
We evaluate, diagnose and treat; if needed, we give you a lab order for a lab near you.
We see adults and children of all ages. Patients under 21: a parent with patria potestad (parental authority) or a legal guardian is on camera for the whole visit and shows photo ID.
Real-time video visit. We send you a secure video link. It works on your phone, tablet or computer.
If appropriate, the doctor prescribes and we send your prescription to your pharmacy.
You must be physically in Puerto Rico during the visit (visitors, tourists and cruise passengers are welcome).
In an emergency, call 911.
