Caribbean wellness retreat: why Puerto Rico, and what it is (and is not)
A Caribbean wellness retreat is a short stay on an island to rest, move and rebuild habits. Puerto Rico is a US territory, so a US citizen needs no passport to travel from the mainland. THI’s retreat is a small private group with a physician assessment, held in English. It is wellness, not treatment, and not a resort.

You need to stop, but not at a resort
You have been saying for months that you will rest. The calendar fills, the phone never goes quiet, and your days off go to errands and catching up. On any Sunday you are already thinking about Monday.
Carrying on like this has a cost that never shows up on a bill: you sleep in pieces, you move less, you eat whatever is nearby, and the doctor’s appointment waits another month. A long weekend does not fix that, and neither does a beach trip with the phone in your hand.
So you may be searching for a Caribbean wellness retreat: a real pause, with sun and water, but without the logistics of an international trip. There is an option that fits. It is a short stay, in a small group, with a licensed physician involved, on an island a US citizen reaches without a passport. That stay is what we explain here. The retreat is held in English.
Why Puerto Rico among wellness retreats in the Caribbean
Many Caribbean destinations are beautiful, but each comes with its own entry rules, currency and paperwork. Puerto Rico is different in practical ways: it is a US territory. According to USAGov, US citizens do not need a passport to travel between the US and Puerto Rico. You still need valid ID to fly; read the travel guide from US cities for the details. If you are not a US citizen, US entry rules apply.
From the mainland there are nonstop flights to San Juan from a number of cities; check with your airline for the one nearest you. We do not publish flight times or fares because they change.
That is why many people looking for a health retreat in the Caribbean, or just a Caribbean retreat that does not complicate the trip, end up looking at Puerto Rico. It is not the only island with good beaches. It is the one that asks the least paperwork of a traveller from the US, which leaves more energy for what matters: resting.
The retreat is held at a hacienda in Río Grande, Puerto Rico. Río Grande sits next to El Yunque National Forest; read the guide to El Yunque and Río Grande. If the season matters to you, read Puerto Rico in November.
Being part of the US makes some things simpler: a language many travellers already use, and a licensed physician in the territory itself. What does not change is that you are travelling to an island: plan your medicines and your ID ahead of time.
A physician on the team: the difference from resort wellness
Many wellness retreats in the Caribbean run inside a resort: a spa, classes, a buffet and an open schedule. That is a good way to rest, and there is nothing wrong with it. But it usually does not include a doctor who looks at your history and decides whether lab work makes sense.
THI’s retreat does. Dra. Diana Nieves Castro, a general practice physician licensed in Puerto Rico, does a physician assessment and orders labs when clinically appropriate. We do not promise specific tests or results. The approach is root-cause: questions, an exam, and a plan of habits you can keep when you get home.
To see what the medical side covers, read the physician visit and labs. To compare with a spa or resort, read medical and wellness retreat vs spa resort, or the general guide to the medical and wellness retreat.
The retreat does not replace your own doctor, who stays in charge of your care. It is a wellness stay, not treatment.
How the days work
THI runs an in-person wellness retreat in Puerto Rico: 5 days starting November 15, 2026, with 10 places, at $3,500 per person. A new group starts on the 1st and the 15th of every month from that first date. Applications for the first group close November 5, 2026.
Here is what a typical day looks like. It is not a promised schedule; see what to expect at the retreat for more.
A typical day, not a fixed schedule (we share the full schedule with accepted guests): gentle morning movement such as gentle stretching and mobility, breakfast together, a medical or education session, free time or an island activity, dinner, and an evening meditation, with phones kept away during sessions and meals.
Included in the price:
- Lodging and meals
- A physician assessment, and labs when clinically appropriate
- Mobility coaching, meditation and breathwork, and nutrition sessions
- Island activities and airport transfers
A small private-group stay, not a resort
It is worth being plain about this: it is not a resort. It is a private stay for a small group, at a hacienda in Río Grande, Puerto Rico. There is no pool bar, no casino and no hundreds of other guests.
Simple, comfortable lodging at the hacienda. Room details, including whether rooms are shared or private, are confirmed in your acceptance email.
What you gain: close attention from the physician and the people who lead the sessions, a rhythm that makes sense from day one, and people who are there for the same reason. What you give up: the variety and the anonymity of a big resort. If you prefer the second, a resort may suit you better, and we would tell you so.
The exact location is shared with confirmed guests. We do not publish more detail than you see here.
The honest questions
Is it worth it? It depends on what you want. If you want a few days on a beach, there are simpler options. If you want a structured pause, with a physician assessment and a plan for going home, it is a different thing. For the breakdown, read what a wellness retreat includes and costs.
I am not a spiritual or yoga person. You do not have to be. The sessions are mobility, breathwork, meditation and nutrition, and you take part at the level that feels right.
Is it safe with my health condition? Often yes, but talk to us first if you are pregnant, had recent surgery, have a condition that is not stable, or need mobility support. That is not a list of people who cannot come; it is so we can plan your week safely.
Can I spare the time? It is 5 days. Many people find a short block easier to protect on the calendar than two weeks.
To see whether it fits you, read who the retreat is for. It is open to everyone.
The first step
If a Caribbean wellness getaway has been on your list for a while, this is a way to make it concrete without a long planning project. Nobody needs to decide everything today. You read, you ask what you want to ask, and you apply when you are ready.
The first step is small: you reserve your place on the retreat page. Fill in the short form and pay the deposit through secure checkout. Our team then writes to you, and our physician reviews your health in a private step. If the retreat is not a good fit for you, we refund your deposit in full. The steps are in how to apply to the retreat.
If you are going through a hard time with your mental health, a retreat is not the answer: call or text 988, or call Línea PAS at 1-800-981-0023.
How we care for you
We evaluate, diagnose and treat; if needed, we give you a lab order for a lab near you.
We see adults and children of all ages. Patients under 21: a parent with patria potestad (parental authority) or a legal guardian is on camera for the whole visit and shows photo ID.
Real-time video visit. We send you a secure video link. It works on your phone, tablet or computer.
If appropriate, the doctor prescribes and we send your prescription to your pharmacy.
You must be physically in Puerto Rico during the visit (visitors, tourists and cruise passengers are welcome).
In an emergency, call 911.
