Beach and island retreat in Puerto Rico: the sea as part of the rest, not the excuse

A beach or island retreat is a short stay by the sea to rest and rebuild habits. THI’s is in Puerto Rico: a small group, 5 days, with time on the island and a physician assessment from a licensed physician. It is wellness, not treatment, and it is not a resort or a party trip. It is held in English.

Is this an emergency?

This is not an emergency service. Call 911 or go to the nearest emergency room if you have:

  • Chest pain, shortness of breath or fainting: call 911, do not wait to travel
  • A recent change in a health condition, or a new medicine, before you travel: talk to your own doctor first
  • Thoughts of harming yourself or ending your life: call or text 988, or call Línea PAS at 1-800-981-0023

Picture the sea. Now picture the phone staying in the room

It is Tuesday afternoon and you are looking at a screen. A photo of a beach has been your desktop background for two years. Every time you see it, you think the same thing: this year.

The beach was never the problem. The last time you had a few days, you spent them with the phone in your hand, answering messages beside the water. You came home tanned and just as tired. The sea was there, but you were not.

That may be why you are searching for a beach retreat or an island retreat. You do not want more sun. You want someone to take away the excuse to stay connected: a short stay, a small group and a physician who looks at your health while you slow down. That is what we explain here. The retreat is held in English.

Why an island helps you rest

There is a practical reason so many people look for an island when they need to stop: the journey is part of the break. When you board a plane and step off somewhere else, your body understands the routine stayed behind. At home, every pause competes with email, the kitchen and the couch.

Puerto Rico adds something that makes the trip easier from the mainland: it is a US territory. According to USAGov, US citizens do not need a passport to travel between the US and Puerto Rico. You still need valid ID to fly. To plan the trip, read the travel guide from US cities.

The retreat is held at a hacienda in Río Grande, Puerto Rico. Río Grande is on the island’s northeast coast, next to El Yunque National Forest. If you are drawn more to green than to blue, read about the nature retreat or the rainforest retreat.

What we do not do is promise you a beach outside your room or a sea that behaves a certain way. We do not publish the property’s name or address, and sea conditions change from one day to the next. What we do is describe what is in the program.

The sea inside the program, not instead of it

The days have time outdoors, and the list below is what can be done in the area and across the island. These are possibilities, not a promised program: what your group does depends on the itinerary for your start date, the season, the weather, sea conditions and your physician assessment. Every activity is optional.

Those activities sit alongside sessions of mobility, breathwork, meditation and nutrition, and a physician assessment. Here is what a typical day looks like. It is not a promised schedule; see what to expect at the retreat for more.

A typical day, not a fixed schedule (we share the full schedule with accepted guests): gentle morning movement such as gentle stretching and mobility, breakfast together, a medical or education session, free time or an island activity, dinner, and an evening meditation, with phones kept away during sessions and meals.

If you want more movement and water, read the adventure wellness retreat.

  • Snorkeling
  • Kayaking or paddleboarding on calm water
  • Beach time and swimming
  • A walk in El Yunque rainforest
  • A visit to a waterfall or river pool, depending on conditions
  • Birdwatching and wildlife walks, such as listening for coquí frogs at night
  • An evening trip to a bioluminescent bay, depending on the season and the moon
  • A boat trip to a cay or small island
  • A gentle adventure activity, such as a zip line, for those who want it
  • A horseback ride
  • A visit to a coffee or cacao farm
  • A Puerto Rican cooking class or a dance class
  • An afternoon in Old San Juan
  • An evening of stargazing
  • Gentle outdoor movement in the morning

Not every activity runs in every group. We confirm your group's itinerary with accepted guests, and every activity is optional.

How the retreat works

THI runs an in-person wellness retreat in Puerto Rico: 5 days starting November 15, 2026, with 10 places, at $3,500 per person. Applications for the first group close November 5, 2026.

Included in the price:

  • Lodging and meals
  • A physician assessment, and labs when clinically appropriate
  • Mobility coaching, meditation and breathwork, and nutrition sessions
  • Island activities and airport transfers

A small private-group stay, not a resort or a party trip

It is worth being plain. It is a private stay for a small group of 10 places, at a hacienda in Río Grande, Puerto Rico, not a large resort.

Simple, comfortable lodging at the hacienda. Room details, including whether rooms are shared or private, are confirmed in your acceptance email.

What you gain: close attention from the physician and the people who lead the sessions, a rhythm that makes sense from day one, and people who are there for the same reason. What you give up: the variety and the anonymity of a big resort. If you prefer the second, a resort may suit you better, and we would tell you so.

If you want to compare it with other ways to spend a few days in the Caribbean, read the guide to the Caribbean wellness retreat.

The sea, with care

A stay by the sea asks for common sense, and none of it is special to this retreat: use sunscreen and reapply it, drink water often, rest from the midday sun, and swim or do water activities only within what you know how to do.

If you take medicines, have a heart, skin or breathing condition, or have not exercised in a while, talk to your own doctor before you travel. Tell us what we should know when you reserve; the retreat physician reviews your health in a private step.

If you need mobility support or have access needs, say so on your application and we will work out what is possible.

The honest questions

Is it worth it compared with a few days at the beach? It depends on what you want. If you want a few days on a beach, there are simpler options. If you want a structured pause, with a physician assessment and a plan for going home, it is a different thing. For the breakdown, read what a wellness retreat includes and costs.

I am not a strong swimmer. Activities are confirmed with accepted guests, and you take part at the level that feels right. Say so on your application.

What about the time of year? To see what the island is like in other months, read Puerto Rico in November or the winter retreat.

What if it is not what I expected? If we do not accept you, you pay nothing, and if the retreat is not a good fit for you, we refund your deposit in full. The terms are on the retreat page.

The first step

The beach photo has been on your desktop for two years. Nobody needs to decide everything today. You read, you ask what you want to ask, and you reserve when you are ready.

The first step is small: you reserve your place on the retreat page. Fill in the short form and pay the deposit through secure checkout. Our team then writes to you, and our physician reviews your health in a private step. The steps are in how to apply to the retreat.

If you are going through a hard time with your mental health, a retreat is not the answer: call or text 988, or call Línea PAS at 1-800-981-0023.

How we care for you

  • We evaluate, diagnose and treat; if needed, we give you a lab order for a lab near you.

  • We see adults and children of all ages. Patients under 21: a parent with patria potestad (parental authority) or a legal guardian is on camera for the whole visit and shows photo ID.

  • Real-time video visit. We send you a secure video link. It works on your phone, tablet or computer.

  • If appropriate, the doctor prescribes and we send your prescription to your pharmacy.

  • You must be physically in Puerto Rico during the visit (visitors, tourists and cruise passengers are welcome).

  • In an emergency, call 911.
