Holistic wellness retreat in Puerto Rico: body, mind and habits together

A holistic wellness retreat looks after body, mind and daily habits together, rather than just one of them. The Health Initiative’s retreat runs 5 days in Puerto Rico for a small group and includes a physician assessment, with labs when clinically appropriate. It is held in English. It is not medical treatment and does not replace your own doctor.

When you want to feel better and don’t know where to start

You may know the picture. You wake up tired even though you slept. You have a list of things you know you should do: sleep more, walk, eat better, stop for a while. And every week work, family and your phone run over the list.

A holistic wellness retreat starts from a simple idea: tiredness, stress and habits that don’t stick rarely have one cause, so they rarely have one fix. A massage doesn’t change your sleep. A meditation session doesn’t look at your bloodwork. An exercise plan doesn’t give you time to stop.

What usually happens is that we try one thing at a time: a sleep app one month, a gym membership the next. Each attempt starts from zero, nobody looks at the whole picture, and within a few weeks the routine is back to what it was. That is not a lack of willpower. No single change holds up against a full life.

Staying as you are has a cost too. Every month you change nothing, you carry the same load, plus the feeling that you should have started sooner. You don’t need a crisis to give yourself permission to stop for a few days and look at the whole picture, with someone who knows health beside you.

What holistic means, and what it doesn’t

The word is used loosely. Here it has a specific meaning. The World Health Organization defines health as a state of complete physical, mental and social well-being, not merely the absence of disease. A holistic retreat, properly understood, is one that looks at those parts at the same time.

The parts do affect each other. Poor sleep makes stress harder to carry. Stress changes what and when you eat. Eating irregularly and sitting all day change how your body feels, and a body that aches makes it harder to rest. When each piece is handled by a different person on a different day, nobody sees how they fit together, and you are left to connect them yourself.

That sets it apart from two things that look similar:

  • A purely spiritual retreat. It can give you calm and meaning, but it rarely includes a physician who asks about your sleep, your medicines or your labs.
  • A purely spa retreat. It can give you rest and comfort, but it ends when the weekend ends, and it often leaves you without habits you can keep.
  • A holistic retreat with a physician. It combines rest, movement, breathwork and nutrition with a medical assessment, so the plan you take home starts from what is actually going on with you.

What this holistic retreat includes and how it feels

The Health Initiative (THI) runs an in-person medical and wellness retreat in Puerto Rico: 5 days starting November 15, 2026, with 10 places, at $3,500 per person. It is held at a hacienda in Río Grande, Puerto Rico. The retreat is held in English and is open to everyone. A new group starts on the 1st and the 15th of every month from the first start date.

The group is small on purpose. With few people, the physician, the coaches and the rest of the group know who you are, and the sessions don’t feel like a mass class. What you gain is close attention; what we don’t promise is an agenda crowded with extras.

Included in the price:

  • A physician assessment, and labs when clinically appropriate, with Dra. Diana Nieves Castro, a general practice physician licensed in Puerto Rico
  • Mobility coaching
  • Meditation and breathwork sessions
  • Nutrition sessions
  • Lodging and meals
  • Island activities and airport transfers

What a day looks like, and what the medical side involves

Here is a typical day, not a promised schedule. For more, read what to expect at the retreat.

A typical day, not a fixed schedule (we share the full schedule with accepted guests): gentle morning movement such as gentle stretching and mobility, breakfast together, a medical or education session, free time or an island activity, dinner, and an evening meditation, with phones kept away during sessions and meals.

Here is what the medical side includes:

A physician assessment with Dra. Diana Nieves Castro, MD, and lab work when it is clinically appropriate. She provides all the medical care at the retreat.

We do not promise specific tests or results: labs are ordered when the physician judges them clinically appropriate. To see how that visit works, read the physician visit and labs at the retreat.

Honest questions before you decide

Is it worth it? That depends on what you want. If you expect a few days to undo years of habits, no. If you want a real pause, a medical check-in and a concrete plan for going back, it is a reasonable use of your time. For the price breakdown, read what a wellness retreat includes and costs.

I’m not a spiritual or yoga person. You don’t need to be. The sessions are mobility, breathwork, guided meditation and nutrition, explained in practical terms. You take part in your own way, and you can rest and do nothing when you need to. See also who the retreat is for.

I can’t afford the time. It is a short break, and the trip can be planned well ahead. Read travelling from US cities and how to apply.

Will it change my life? We would rather be straight with you: no stay can promise that, and we do not promise outcomes. What it can do is give you a few quiet days, a conversation with a physician about your health, and a short list of changes that fit your actual life. What you do with that list after you get home is the part that counts.

Is it safe with my condition? If you are pregnant, had recent surgery, had a recent hospital stay, or have a condition that isn’t stable, talk to us before you apply. That does not mean you cannot come; it means we want to plan your week safely. The retreat does not replace your own doctor.

If you are going through a hard time with your mental health, a retreat is not the place for that: talk to a professional. In a crisis, call or text [988](tel:988) (it works in Puerto Rico and on the mainland; press 2 for Spanish), call Línea PAS at 1-800-981-0023, or call 911.

What you take home: habits you can keep

The strongest evidence on wellbeing is unglamorous. The CDC recommends that adults 18 to 60 sleep 7 or more hours a night and keep the same bed and wake times. It also recommends 150 minutes of moderate activity a week plus muscle-strengthening on 2 days, and says high-quality relationships support health and stress management.

A holistic retreat is useful if it helps you put those into practice. Pick one or two habits to keep, not a long list, and put them in your calendar before your first day back. See how to reset your sleep schedule and how to start with guided meditation.

It helps to decide before you arrive what you want to leave with. Maybe it is a bedtime you can keep, a walk you can fit in on weekdays, a clearer idea of what to ask your own doctor, or simply the feeling of having been looked after for a few days. None of these is dramatic, and that is the point: small changes that you repeat are the ones that last.

Share what the physician found with your own doctor, and follow up on any lab results with them. You can also read the guide to the medical wellness retreat, or the ones on November in Puerto Rico and the El Yunque and Río Grande area.

Frequently asked questions

What is a holistic wellness retreat?

A short stay that looks after body, mind and daily habits together. THI’s retreat combines rest, mobility coaching, meditation and breathwork, nutrition sessions and a physician assessment. It is not treatment.

How is a holistic retreat different from a spa retreat?

A spa retreat centres on rest and pampering. A holistic one, like THI’s, adds mobility coaching, meditation, nutrition and a physician assessment, so you leave with a plan of habits. Neither replaces your own doctor.

Does a holistic health retreat include labs?

THI’s retreat includes a physician assessment and labs when clinically appropriate, with a general practice physician licensed in Puerto Rico. We do not promise specific tests or results.

Do I need to be spiritual or do yoga?

No. The sessions are mobility, breathwork, guided meditation and nutrition. You take part in your own way and can rest when you need to.

Is the retreat medical treatment?

No. It is wellness: assessment, education, habits and coaching. It does not replace your own doctor, who stays in charge of your care.

Is the retreat held in English?

Yes. The retreat is held in English, and the sessions are in English. Dra. Diana Nieves Castro speaks English and Spanish.

What should I do if I feel hopeless?

Talk to someone today. Call or text 988, which works in Puerto Rico and on the mainland, or call Línea PAS at 1-800-981-0023. If you are in immediate danger, call 911.
