Holistic wellness retreat in Puerto Rico: the physician assessment first, the rest after
A holistic wellness retreat combines rest, movement, breathwork, meditation and nutrition in one stay. The Health Initiative’s starts with a physician assessment by a general practice physician licensed in Puerto Rico, then moves into sessions at a hacienda in Río Grande. It is wellness, not treatment, and it does not replace your own doctor. It is held in English.

You already try things, but nobody looks at them together
Perhaps you already do some of it: a tea before bed, a supplement a friend recommended, ten minutes of breathing from an app, a yoga class when the calendar allows. These are sincere attempts to take care of yourself. The trouble is that each one lives on its own, and nobody has the whole picture.
Meanwhile the symptoms that worry you are still there. The doctor’s appointment keeps slipping because you want to "try something natural" first. That wait has a cost: months without knowing whether what you feel has a cause that can be looked at, measured and addressed. If a symptom worries you, see your doctor first; the retreat is not the place to sort it out and does not replace that visit.
A well-run holistic wellness retreat does not compete with medicine. It starts with it. First a physician asks about your history, what you take and what worries you; then you spend a few days moving, breathing, resting and eating with intention. That is what we explain here, including what the retreat is not. The retreat is held in English.
What we mean by "holistic"
The word is used for nearly everything, so it helps to be concrete. Here it means that body, mind and habits are looked at in the same week by the same team: sleep, food, movement, stress and what you take.
It does not mean we blend "natural medicine" with medicine. The retreat’s physician is a general practice physician licensed in Puerto Rico: a medical assessment first, then sleep, nutrition, movement and stress, and a review of everything you take. The retreat is a wellness stay, not medical treatment, and the physician assessment is not ongoing care.
If you want the bigger idea, the guide to the holistic wellness retreat explains how body, mind and habits relate. This guide is for the person who wants to know, plainly, what happens in practice.
How the stay is ordered
The Health Initiative (THI) runs an in-person wellness retreat at a hacienda in Río Grande, Puerto Rico: 5 days starting November 15, 2026, with 10 places, at $3,500 per person. It is held at a hacienda in Río Grande, Puerto Rico. Reservations for the first group close November 5, 2026.
The order matters more than the list:
- Physician assessment. Dr. Diana Nieves Castro, MD, a general practice physician licensed in Puerto Rico, reviews your history and your medicines, and orders labs when clinically appropriate. It is the starting point for the rest.
- Movement. Mobility coaching at a level you can sustain, not high-performance training.
- Breathwork and meditation. Sessions to practice simple tools for attention and rest. They are offered as wellness, not as a treatment for any disease.
- Nutrition. Sessions on how to organize meals you can repeat when you get home.
- Nature and rest. Time to slow down, step away from the phone and, depending on the group’s itinerary, activities on the island.
Before you come: tell us what we should know
Tell us before you travel if you are pregnant, or have a heart or lung condition, seizures, panic attacks or a history of trauma, a history of an eating disorder, injuries or diabetes. A change in how you eat can affect people who use diabetes medicines, and not every exercise, breathing or meditation session suits every person.
Every session is optional and you can stop at any time. The physician assessment decides with you what suits you.
What a day looks like and what you will eat
We do not publish a day-by-day schedule because the itinerary depends on each group. This is what we can tell you:
A typical day, not a fixed schedule (we share the full schedule with accepted guests): gentle morning movement such as gentle stretching and mobility, breakfast together, a medical or education session, free time or an island activity, dinner, and an evening meditation, with phones kept away during sessions and meals.
Fresh, home-style meals built around whole foods, with Puerto Rican flavors. Tell us about allergies and food preferences on your application.
Island activities are possibilities, not promises, and depend on the group’s itinerary:
Examples of what the area offers and what the days may include: snorkeling, a walk in El Yunque rainforest, time at Luquillo beach, a gentle stretching session, and relaxed evenings outdoors. The activities for your cohort are confirmed with accepted guests.
For more detail, read what to expect at the retreat and the guide to El Yunque and Río Grande.
What the retreat is, and what it is not
If you come from a world where "holistic" sometimes means a product counter, this part is for you. The retreat’s program is the physician assessment and sessions in mobility, breathwork, meditation and nutrition, plus rest and activities depending on the itinerary. Nothing else is part of the program. Specifically:
- It is wellness, not treatment. We do not promise that any practice cures, treats or prevents a disease.
- It is not alternative medicine. The program does not include alternative therapies, and your liver and kidneys already do the work that detox products promise, for which there is no good evidence. Time to put the phone away is a different thing.
- It is a place to talk about what you take. If you use a therapy with a licensed practitioner, tell us, tell your prescriber and keep any prescribed treatment.
What the evidence says, honestly
Not every wellness practice has the same backing, and it is better to say so.
The evidence is stronger for the basics: enough sleep, regular movement and an eating pattern you can keep. Those are the habits the retreat gives you time to practice.
For breathwork and meditation the evidence is more limited and mixed: the evidence is limited and mixed. They can sit alongside medical care; they do not replace it.
For supplements and herbs the evidence is limited or mixed in most cases, and dietary supplements are not approved to treat, cure or prevent disease. "Natural" does not mean safe.
For "detox" products the evidence is limited.
If you take supplements, herbs or medicines: bring the list
Supplements and herbs can have side effects and can interact with prescription and over-the-counter medicines, especially blood thinners, blood pressure and diabetes medicines, antidepressants and medicines used around anesthesia or surgery. They need extra caution in pregnancy, breastfeeding and children.
So we ask for something simple before you travel: write down everything you take, including prescriptions, over-the-counter medicines, supplements, herbs and teas, with the dose if you know it. The physician reviews it at the physician assessment. It is a safety check, not a judgment of your choices.
Do not stop or change a prescribed medicine on your own in order to come. If you have a question, talk first to whoever prescribed it. For conditions that need follow-up, read travelling to the retreat with a chronic condition.
How it differs from a spa or a purely spiritual retreat
A spa gives you rest and comfort. A spiritual retreat can give you calm and meaning. Both are legitimate. What they usually do not have is a physician who looks at your history, your medicines and, when appropriate, your lab work before the rest is planned. Picture arriving on a Monday with your medicine list in hand: before the first session, someone goes through it with you and tells you which sessions to take gently.
THI’s retreat does not replace your own doctor, who stays in charge of your care. It is a wellness stay with a physician on the team, not ongoing medical care. To compare options, read medical and wellness retreat vs spa resort and how to choose a wellness retreat.
The honest questions
I am not a spiritual person. You do not have to be. The sessions are mobility, breathwork, meditation and nutrition, and you take part at the level that feels right.
I am skeptical about "holistic". That is a reasonable stance, and this page tries to earn your trust by saying what we do not do and how strong the evidence is.
I take medicines. Tell us before you travel so the physician can review them and we can plan your week; read who the retreat is for.
I have mobility or dietary needs. Read accessibility and dietary needs and tell us when you reserve.
What should I bring? Comfortable clothes for movement, swimwear, sun protection, a light rain jacket, walking shoes, a refillable water bottle, and any medications you take. We send the full list with your acceptance. The details are in the packing list.
What does it cost and what is included? The breakdown is in what a wellness retreat includes and costs.
When not to wait for the retreat
A retreat is not the place to sort out a symptom that worries you. If you have chest pain, shortness of breath, fainting, sudden weakness on one side of the body or a severe allergic reaction, call 911. If you have had a symptom you cannot explain for weeks, unexplained weight loss, bleeding or a change in your mood, see your doctor before you travel and do not try remedies while you wait.
If you are going through a hard time with your mental health, a retreat is not the answer: call or text 988, or call Línea PAS at 1-800-981-0023.
The first step
Nobody needs to decide everything today. You read, you ask what you want to ask, and when you are ready you reserve your place on the retreat page with a deposit through secure checkout. Our team then writes to you, and the physician reviews your health in a private step. The steps are in how to reserve your place at the retreat.
In the meantime, do one thing: write down everything you take. It will help at the retreat and at any appointment with any clinician.
How we care for you
We evaluate, diagnose and treat; if needed, we give you a lab order for a lab near you.
We see adults and children of all ages. Patients under 21: a parent with patria potestad (parental authority) or a legal guardian is on camera for the whole visit and shows photo ID.
Real-time video visit. We send you a secure video link. It works on your phone, tablet or computer.
If appropriate, the doctor prescribes and we send your prescription to your pharmacy.
You must be physically in Puerto Rico during the visit (visitors, tourists and cruise passengers are welcome).
In an emergency, call 911.
