Self-care retreat: days away for people who care for everyone else
A self-care retreat is a short, structured stay away from home to rest, move, eat well and rebuild habits that fall apart in the routine. THI’s runs 5 days in Puerto Rico, with a small group and a licensed physician, held in English. It is wellness, not treatment.

The hot bath and the candle were not enough
You tried. You ran the bath, lit the candle, promised yourself an hour. Ten minutes in, you were thinking about tomorrow’s list, about who needs what, and about what is sitting unanswered on your phone.
That is not a lack of willpower. Routine self-care competes with everything else in the same week, and it loses. You look after the job, the house, the kids, the parents, the team. Your own rest waits until everything else is settled, and it almost never is.
Carrying on like this has a cost that never shows up on a bill: sleep in pieces, a body that moves less, meals on the run, the check-up that waits another month. Each week makes it feel more normal. A self-care retreat exists to break that pattern with something other than one more good intention: 5 structured days, away from the house you always have. The retreat is held in English.
Why a block of days away, and not a weekend
A self-care weekend at home has three problems. You are still in the same place, with the same knocks on the door. Nobody else organises the day, so you decide everything, which is exactly what already wears you out. And it ends on Sunday night, before your body has slowed down.
A block of 5 days has a different shape. A small group, prepared meals and a different setting take some of the daily decisions off your plate. And 5 days gives more time than a weekend, which ends on Sunday night.
We do not promise a result or a specific change. What we can describe is the structure: protected time and a different setting, with possible activities that depend on each group’s itinerary (see the typical day below). Read what to expect at the retreat to see how the days are organised.
This is not a holiday under another name. For the difference, read wellness retreat vs vacation.
Who it is for: people who care for everyone else
It is open to everyone. But this angle is for a particular person: the one who holds things up for others and leaves their own care for last.
You may recognise yourself in one of these:
- You look after a parent or a child, and your calendar revolves around theirs
- You run a team, a business or a household, and you are the person everyone comes to
- You work in a helping profession (health, education, service) and get home with nothing left to give
- You have been saying for years that you will get back to exercise, sleep or a check-up, and every week you put it off again
What self-care means here, concretely
On a retreat, self-care is not a list of treats. It is a set of simple things, done with attention and with support:
Dra. Diana Nieves Castro, a general practice physician licensed in Puerto Rico, does a physician assessment and orders labs when clinically appropriate. We do not promise specific tests or results. You receive an assessment and can ask your questions. Read the physician visit and labs.
Mobility, meditation and breathwork, and nutrition sessions, at a level that feels right to you. Meals prepared for you. And island activities that depend on each group’s itinerary; for the possible ones, read what to expect at the retreat. None of it replaces your own doctor.
Island activities that are possible
Activities depend on each group’s itinerary and are not guaranteed. These are some of the possible ones.
- Snorkeling
- Kayaking or paddleboarding on calm water
- Beach time and swimming
- A walk in El Yunque rainforest
- A visit to a waterfall or river pool, depending on conditions
- Birdwatching and wildlife walks, such as listening for coquí frogs at night
- An evening trip to a bioluminescent bay, depending on the season and the moon
- A boat trip to a cay or small island
- A gentle adventure activity, such as a zip line, for those who want it
- A horseback ride
- A visit to a coffee or cacao farm
- A Puerto Rican cooking class or a dance class
- An afternoon in Old San Juan
- An evening of stargazing
- Gentle outdoor movement in the morning
Not every activity runs in every group. We confirm your group's itinerary with accepted guests, and every activity is optional.
How the days work
THI runs an in-person wellness retreat at a hacienda in Río Grande, Puerto Rico: 5 days starting November 15, 2026, with 10 places, at $3,500 per person. There is more than one group; the dates are on the retreat page. Applications for the first group close November 5, 2026.
Here is what a typical day looks like. It is not a promised schedule:
A typical day, not a fixed schedule (we share the full schedule with accepted guests): gentle morning movement such as gentle stretching and mobility, breakfast together, a medical or education session, free time or an island activity, dinner, and an evening meditation, with phones kept away during sessions and meals.
Included in the price:
- Lodging and meals
- A physician assessment, and labs when clinically appropriate
- Mobility coaching, meditation and breathwork, and nutrition sessions
- Island activities and airport transfers
The honest questions
I feel guilty leaving. It is the most common doubt, and it makes sense: if you are the one who holds everyone up, leaving feels like abandoning them. 5 days is not abandoning. Plan who covers what and give notice; looking after yourself is part of looking after others.
I cannot spare the time or the money. That is a real decision and we do not minimise it. The price is $3,500 per person. For the breakdown and how to compare, read what a wellness retreat includes and costs.
I am not a yoga or spiritual person. You do not have to be. You take part at the level that feels right; read who the retreat is for.
What if my health or mood is not good? Talk to us first if you have a health condition; read retreat with a chronic condition: talk to us first. If you are going through a hard time with your mental health, a retreat is not the answer: call or text 988.
Is this treatment? No. It is wellness: assessment, education, habits and coaching.
Reserving your place is the first act of self-care
Self-care almost never starts with the hot bath. It starts with a decision that goes on the calendar before someone else fills the space. That is what reserving is: saying that week is yours.
The first step is small: you reserve your place on the retreat page. Fill in the short form and pay the deposit through secure checkout. The deposit, cancellation and transfer terms are on the retreat page. The steps are in how to apply to the retreat. The only urgency is a real one: applications close on the date shown, and each group has 10 places.
If you are going through a hard time with your mental health, call or text 988, or call Línea PAS at 1-800-981-0023.
How we care for you
We evaluate, diagnose and treat; if needed, we give you a lab order for a lab near you.
We see adults and children of all ages. Patients under 21: a parent with patria potestad (parental authority) or a legal guardian is on camera for the whole visit and shows photo ID.
Real-time video visit. We send you a secure video link. It works on your phone, tablet or computer.
If appropriate, the doctor prescribes and we send your prescription to your pharmacy.
You must be physically in Puerto Rico during the visit (visitors, tourists and cruise passengers are welcome).
In an emergency, call 911.
