Mental wellness retreat: what it is, what it is not, and who should talk to a clinician first

A mental wellness retreat is a short break from the daily load to rest, spend time in nature and turn the noise down. THI’s is 5 days in Puerto Rico, in a small group, with a licensed physician on the team. It is wellness, not mental health treatment, and it does not replace a therapist, a psychiatrist or your own doctor. It is held in English.

Is it an emergency? Call 911.

Is this an emergency?

This is not an emergency service. Call 911 or go to the nearest emergency room if you have:

  • Thoughts of harming yourself or ending your life: call or text 988, or call Línea PAS at 1-800-981-0023 (24 hours)
  • Low mood, or loss of interest in what you used to enjoy, most days for two weeks or more: talk to a health professional before planning a trip
  • Panic attacks, or feeling that you cannot cope with daily life: talk to a health professional
  • Chest pain, fainting or shortness of breath: call 911

The mind that will not switch off

It is eleven at night and the workday ended hours ago, but you are still replaying this afternoon’s conversation, tomorrow’s list and the message you forgot to answer. You lie down, and your head does not lie down with you.

Carrying on like this has a cost that rarely gets named: you sleep in pieces, you are only half there for the people you love, and every pause you promise yourself waits until the load “eases up”. The load does not ease on its own. A long weekend with the phone in your hand does not ease it either, because your mind travels with you.

That may be why you are searching for a mental wellness retreat: not a cure and not treatment, but 5 days with nature, quiet and a simple rhythm, where activities are optional and you take part at your own level. That is what we explain here, including the part that sounds least like a brochure: what a retreat does not do, and who should talk to a professional first. The retreat is held in English.

What a mental wellness retreat is, and is not

It needs saying plainly at the start. THI’s retreat is not mental health treatment. There is no therapy, no diagnosis, and no promise that you will leave without anxiety, sadness or stress. Anyone who offers you that is promising something a short stay cannot deliver.

What it is: 5 days to set the pace of daily life down for a while. Sleeping somewhere quiet, eating slowly, walking, breathing with attention, being outdoors, and talking if you feel like it, or not. It is rest with just enough structure that you have less to decide.

For you, that might simply be a breather. It is not a replacement for therapy, a medicine or follow-up with your own doctor, and anyone already receiving that care should keep it.

Before you reserve: who should talk to a professional first

A retreat is a bad idea as a substitute for help a person needs right now. Talk to a health professional before planning a trip if any of these is true for you:

  • You have had low mood, or no interest in what you used to enjoy, most days for two weeks or more
  • You have panic attacks, or feel you cannot cope with daily life
  • You have thoughts of harming yourself or ending your life (call or text 988 now, without waiting)
  • There has been a recent change in a mental health diagnosis, or you recently started, changed or stopped a medicine
  • You are going through grief, a crisis or a situation that needs ongoing support, not 5 days

If you need help right now

If you are in danger or thinking of harming yourself, do not wait for a retreat or an appointment. Call or text 988 (the Suicide & Crisis Lifeline), or call Línea PAS at 1-800-981-0023, which answers 24 hours a day in Puerto Rico. If it is a medical emergency, call 911.

If you are in treatment, talk to your clinician before you travel: they know your situation and can tell you whether this kind of break suits you and when. That conversation is worth more than any web page, this one included.

What the break looks like: rest, nature and quiet

THI runs an in-person wellness retreat in Puerto Rico: 5 days starting November 15, 2026, with 10 places, at $3,500 per person. Applications for the first group close November 5, 2026.

It is held at a hacienda in Río Grande, Puerto Rico. Read the guide to El Yunque and Río Grande and, for the setting, the nature retreat. Activities depend on each group’s itinerary and are possibilities, not a promise:

Examples of what the area offers and what the days may include: snorkeling, a walk in El Yunque rainforest, time at Luquillo beach, a gentle stretching session, and relaxed evenings outdoors. The activities for your cohort are confirmed with accepted guests.

Here is what a typical day looks like. It is not a promised schedule; see what to expect at the retreat.

A typical day, not a fixed schedule (we share the full schedule with accepted guests): gentle morning movement such as gentle stretching and mobility, breakfast together, a medical or education session, free time or an island activity, dinner, and an evening meditation, with phones kept away during sessions and meals.

If the phone is part of the noise, the digital detox guide explains how that is handled. Included in the price:

  • Lodging and meals
  • A physician assessment, and labs when clinically appropriate
  • Mobility coaching, meditation and breathwork, and nutrition sessions
  • Island activities and airport transfers

A physician on the team, and what that means

Dra. Diana Nieves Castro, a general practice physician licensed in Puerto Rico, does a physician assessment of every participant. A physician assessment with Dra. Diana Nieves Castro, MD, and lab work when it is clinically appropriate. She provides all the medical care at the retreat.

That matters at a retreat like this for two reasons. First, there is an assessment and, when clinically appropriate, labs. We do not promise specific tests or results. Second, she can tell you honestly whether this is the right time for you, or whether something else should come first.

She is not your psychiatrist or therapist during the week, and the retreat does not replace your own doctor. To see what the medical side covers, read the physician visit and labs.

The honest questions

Will this fix my anxiety or exhaustion? No, and be wary of anyone who says it will. 5 days do not change your job, your family or the circumstances that weigh on you. They give you rest, nature, and sessions of mobility, breathwork and nutrition. For work exhaustion, read the burnout retreat, and for stress, the stress relief retreat.

I am not spiritual or a meditation person. You do not have to be. The sessions are mobility, breathwork, meditation and nutrition, and you take part at the level that feels right. You can also simply rest.

I will be with strangers. It is a small group of 10 places and conversation is optional. If you are travelling alone, read the solo retreat.

What if I do not feel well during the week? Tell the team. You can slow down or sit an activity out. If it is a crisis, you call 988 or 911, the same as at home.

Can I come with my health condition? Often yes, but talk to us first. Read the retreat with health conditions. To see whether it fits you, read who the retreat is for. It is open to everyone.

Is it worth it? It depends on what you want. For the breakdown, read what a wellness retreat includes and costs.

Nobody needs to decide everything today. You read, you ask what you want to ask, and you apply when you are ready.

The first step is small: you reserve your place on the retreat page. Fill in the short form and pay the deposit through secure checkout. If the retreat is not a good fit for you, we refund your deposit in full. The steps are in how to apply to the retreat.

If you are going through a hard time with your mental health, a retreat is not the answer: call or text 988, or call Línea PAS at 1-800-981-0023.

How we care for you

  • We evaluate, diagnose and treat; if needed, we give you a lab order for a lab near you.

  • We see adults and children of all ages. Patients under 21: a parent with patria potestad (parental authority) or a legal guardian is on camera for the whole visit and shows photo ID.

  • Real-time video visit. We send you a secure video link. It works on your phone, tablet or computer.

  • If appropriate, the doctor prescribes and we send your prescription to your pharmacy.

  • You must be physically in Puerto Rico during the visit (visitors, tourists and cruise passengers are welcome).

  • In an emergency, call 911.
