Wellness retreat in the USA with no passport: Puerto Rico, the Caribbean inside the country

If you are looking for a wellness retreat in the USA that feels like the Caribbean, Puerto Rico is US soil: according to USAGov, a US citizen needs no passport to travel from the mainland. The currency is the US dollar and the flight is domestic. THI’s retreat is 5 days, a small group with a physician assessment, held in English. It is wellness, not treatment.

Is this an emergency?

This is not an emergency service. Call 911 or go to the nearest emergency room if you have:

  • Chest pain, shortness of breath or fainting: call 911, do not wait to travel
  • A recent change in a health condition, or a new medicine, before you travel: talk to your own doctor first
  • Thoughts of harming yourself or ending your life: call or text 988, or call Línea PAS at 1-800-981-0023

You want a real break, without the paperwork of going abroad

You have been thinking about getting away for a few days of rest. Then the list starts: renew the passport, exchange money, check what they ask at the border, find out whether your phone works. By the time the planning is done you have no energy left for the trip, and the break slides to next year.

That is the part almost nobody says out loud: sometimes what stops you from resting is not the lack of time, it is the work it takes to get to the rest. Meanwhile the sleep stays broken, the body stays still and the doctor’s appointment keeps waiting.

If you are searching for a wellness retreat in the USA with sun and water, there is an option that removes most of that paperwork: Puerto Rico. It is US territory, so for a US citizen the trip from the mainland is a domestic flight. The retreat is held in English.

Does Puerto Rico count as the USA?

Yes. Puerto Rico is a US territory, and according to USAGov, US citizens do not need a passport to travel between the United States and Puerto Rico. For someone leaving from New York, Chicago, Atlanta or Dallas, it is a flight within the same country.

In practice that means four things you do not have to prepare: no passport step for the trip, the currency is the US dollar so there is nothing to exchange, the sessions are in English, and calls and texts on most phone plans work as they do at home (confirm yours with your carrier).

What you do need is valid ID to get through airport security. The guide to getting to the retreat from US cities explains what the TSA asks for and what to do if your license is not REAL ID compliant.

What a Puerto Rico retreat brings that a mainland one does not

There are good wellness retreats in Colorado, California and Arizona, and we do not say otherwise. What changes here is the setting: a tropical island with rainforest and sea nearby, one flight away, under the rules of your own country.

The retreat is held at a hacienda in Río Grande, Puerto Rico. Río Grande is a town near El Yunque. Read the guide to El Yunque and Río Grande to learn the surroundings. If the season matters to you, read Puerto Rico in November.

The Caribbean wellness retreat guide compares Puerto Rico with other Caribbean destinations.

With a physician on the team

Dra. Diana Nieves Castro, a general practice physician licensed in Puerto Rico, does a physician assessment and orders labs when clinically appropriate. We do not promise specific tests or results.

The medical side is an assessment with Dra. Nieves Castro, MD, and labs when clinically appropriate. To see what the medical side covers, read the physician visit and labs.

The retreat does not replace your own doctor, who stays in charge of your care. It is a wellness stay, not treatment.

How the days work

THI runs an in-person wellness retreat in Puerto Rico: 5 days starting November 15, 2026, with 10 places, at $3,500 per person. A new group starts on the 1st and the 15th of every month from that first date. Applications for the first group close November 5, 2026.

Here is what a typical day looks like. It is not a promised schedule; see what to expect at the retreat for more.

A typical day, not a fixed schedule (we share the full schedule with accepted guests): gentle morning movement such as gentle stretching and mobility, breakfast together, a medical or education session, free time or an island activity, dinner, and an evening meditation, with phones kept away during sessions and meals.

Included in the price:

  • Lodging and meals
  • A physician assessment, and labs when clinically appropriate
  • Mobility coaching, meditation and breathwork, and nutrition sessions
  • Island activities and airport transfers

What the price covers and what it does not

The $3,500 per person does not list airfare among its inclusions; confirm the details on the retreat page. We do not publish airlines, fares or flight times because they change.

Airport transfers are included. Arrive on November 15, 2026; the retreat lasts 5 days and you depart after the last one. Airport transfers are included, and we confirm arrival and departure details with accepted guests.

For the full breakdown, read what a wellness retreat includes and costs. For what to bring, read the packing list.

If you are not a US citizen

Entry rules depend on your immigration status. We do not summarize them here because they change and because the right source is official. Before you apply, confirm with your airline and with official US government sources which documents you need to fly to Puerto Rico from where you are leaving.

If you already live in the United States under a status that lets you travel within the country, still check your document with the airline. It is better to ask before you buy the ticket than to find out at the airport.

The honest questions

Is it a resort? No. It is a private stay for a small group of 10 places, at a hacienda in Río Grande, Puerto Rico. If you want the variety of a large resort, another option may suit you better, and we would tell you so.

Is it worth it? It depends on what you want. If you want a few days on a beach, there are simpler options. If you want a structured pause, with a physician assessment, it is a different thing. Read the guide to a wellness retreat vs a vacation.

I am not a spiritual or yoga person. You do not have to be. The sessions are mobility, breathwork, meditation and nutrition, and you take part at the level that feels right.

Can I come with a health condition? Tell us on your application: our physician reviews your health in a private step and advises whether the retreat is a fit for you. Read talk to us first if you have a chronic condition.

Can I come alone? Yes, you can apply on your own. Read the guide to a solo wellness retreat.

To see whether it fits you, read who the retreat is for. It is open to everyone.
